REQUIRED DOCUMENTS

FOR MARRIAGE

  1. BAPTISMAL CERTIFICATES (New) with notation “for MARRIAGE PURPOSES”
  2. CONFIRMATION CERTIFICATES (New) with notation “for MARRIAGE PURPOSES”
  3. MARRIAGE LICENSE- applied for from the respective municipal offices of the couple. (License valid for 120 days)
  4. BIRTH CERTIFICATE – NSO Certified true copy.
  5. CENOMAR – Certificate of No Records of Marriage – from NSO
  6. If CIVILLY MARRIED, a certified true copy of Marriage contract from National Statistics Office should be submitted two months before the wedding.

7. MARRIAGE BANNS from the Parish of Presentation of the Child Jesus Parish to bebrought to the respective parishes of the couple and to be posted for three (3) consecutive Sundays prior to the wedding date.

  1. Questionnaire form from the Parish of the Presentation of the Child Jesus Parish to be accomplished by the couple for the canonical interview of the parish priest.
  2. Certification of the seminars taken such as the Pre-Nuptial and the Natural Planning Seminars.
  3. Complete names of all principal sponsors with their respective addresses.
  4. Widowers and widows must present the Death Certificate of their deceased partner.
  5. For Wedding anniversaries, bring a certified copy of your Catholic Marriage contract and come for interview by the Parish Priest/Asst. Parish Priest.
  6. MARRIAGE between CATHOLIC & NON CATHOLIC:

a.) certificate of freedom to marry from the Non-Catholic Minister or Pastor.

b.) PROMISE FOR MIXED MARRIAGE FORM to be filled up during the interview. (to be submitted to the Chancery Office for approval.

  1. MARRIAGE between FILIPINO & FOREIGNER:

a.)Secure a form of clearance from the Chancery Office. Tel. no. 8071662 look for LUCILE

  1. Authorization number and position of officiating priest (Guest Priest)
  2. For foreigners:

a.)A copy of Free Status to Marry.

b.)Clearance from the Chancery office, Tel. # 8071662

2.If you have your own missalette, please give a copy of the draft to the Parish Office for approval two weeks before the wedding.
