Addendum to Offer Letter

The Ohio State University Office of Human Resources Student Employment Policy 10.10 ( governs your employment. The rules, regulations, and policies of The Ohio State University, as well as applicable state and federal law govern your employment. Please familiarize yourself with these.

The Ohio State University is required by federal law to verify the identity and employment authorization of all employees. Accordingly, this offer is contingent upon verification of identity and eligibility to be employed in the United States. You must complete Form I-9, Employment Eligibility Verification () no later than your first day of work. <or> As a current OSU employee, this may be completed by the transfer of a valid I-9 from your current department.[PC1]Please consult with {dept HR contact} for further information regarding this requirement.

In addition, this offer is contingent upon the university’s verification of credentials and other information required by law and/or university policies or practices, including but not limited to a criminal background check. Upon acceptance of this offer, an e-mail will be sent to you with instructions for accessing the on-line background consent and authorization process via the First Advantage website.

Ohio State must comply with the Ohio Ethics Law faculty and staff, including students must receive and acknowledge a copy of the Ohio Ethics Law for public officials and state employees. The Ohio Ethics Law Acknowledgement is included with your new hire paperwork and must be signed and returned to {dept HR contact}.

You will also be required to sign Form SSA1945 (revised 03/14), “Statement Concerning Your Employment in a Job Not Covered by Social Security” when you complete your new hire paperwork.
