Training Grant Nomination Form

Deadline: Noon, Monday, May 9th 2011.

Please email this form with supporting documents (format convertible to pdf) to the student’s BIMS administrator by the submission deadline.

I. Mentor Please complete this form for each Training Grant (TG) nomination and:

 Indicate all of your nominations for this student to TGs: mark the preferred TG.

 Provide a recommendation letter specific to the training grant for each nomination.

 Provide your NIH Bio-sketch with List of Support.

II. Mentor & Nominee Please work together to provide:

 Nominee’s Curriculum Vita (CV).

 Nominee’s research aims: provide a one line description and a one page summary of aims.

 Provide assurance nominee has or will complete curriculum if specified in section IV below.

III. BIMS The BIMS administrator will post to Collab a standard application for each nominee with: 1. this form & recommendation letter(s), 2. Nominee’s CV & Research Aims, 3. Rotation evaluation(s) if available, 4. Current course list, grades, & current GPA, 5. Nominee’s GSAS Application with GRE/MCAT scores, 6. Mentor’s Bio-sketch with list of support.

IV. Additional Requirements Please provide the following additional specifics:

BioDefense, Infectious Diseases, and Immunology: No additional requirements.

Biophysics  Submit 2-3 faculty letters of recommendation  Trainee will take BIOP5060 and will join in Biophysics journal club (Fridays 12-1) and colloquium (Mondays 4-5)

Biotech  Nominee must agree to requirements and apply at: http://faculty.virginia.edu/biotech/Forms.html  Submit 2-3 faculty letters of recommendation

Cardiovascular  Nominee must agree to requirements www.cvrc.virginia.edu/OrientationHandbook.cfm#requirements and apply at www.cvrc.virginia.edu/ApplicationInstructions.cfm

CMB Nominee will provides a summary of the project, and reason for choosing the advisor (<1/2 page). Mentor, please demonstrate that the project is basic science and falls into the category of Cell & Molecular Biology.  Trainees are expected to participate in all CMB activities including Data Dinners, and statistics & research ethics courses.

Cancer  Trainee will provide a paragraph addressing his/her interest in cancer research and how their choice of thesis lab will help attain their training goals (from the student)  Trainee will take Mol Basis of Cancer and Adv Topics in Cancer Research and will join in TG program activities (ie, Cancer Center Seminars)

Pharmacology Trainee will take Statistics and at least one Pharm course (9010, Pharm 9020 or Medical Pharmacology)  Trainee will join Pharmacology journal club (Tuesdays12-1pm) and colloquium (Thurs 4-5pm)
