Phase I: Documentation

(Provide one copy of each document for Phase I)

· LEAD Program application signed by Mentor Principal/Director and Applicant.

· A current résumé aligned to the Florida Principal Leadership Standards describing out-of-classroom leadership roles and experiences.

· A copy of the applicant’s current Florida Teacher’s Certificate.

· Last three (3) End-of-Year Evaluations showing an overall satisfactory rating. Applicants must provide at least one

IPAS from a Broward County Public School. All evaluation documents must show the signature of the Principal/

Designee.

· An unofficial transcript showing a minimum of 15 semester hours of completed coursework toward an Ed. Leadership Master’s Degree/Certification.

Phase II: Leadership Profile Assessment

Application packets scoring at or above a targeted score will prompt a Leadership Profile to be sent directly to the Mentor Principal. The Mentor Principal will score the applicant on a set of leadership indicators.

Phase III: Writing Sample

Applicants with Leadership Profile scores meeting or exceeding a targeted score will be invited to attend a session at HRD to complete a writing sample.

Phase IV: Selection into LEAD

Applicants selected for the LEAD program will be notified in writing prior to the end of the 2010-11 school year.

I have agreed to sponsor this LEAD appl icant and provide leadership experiences so that at the end of the program he/she is a via ble candidate for an administrative position .
