SUBJECT: Perkins Title IC 2009-2010 Online Final Report Templates

The Perkins Title IC 2009-2010 Online Final Report System is not currently accessible. We anticipate no significant form changes and are working with our MIS staff to release the 2009-2010 Perkins IC Online Final Report System as soon as possible.

The district’s Title IC 2009-2010 Final Report package consists of six forms and one optional form to be certified and submitted online by your district Chief Executive Officer (CEO). To help you begin preparing the required Final Report documents, you may use the last year’s generic Final Report templates. When the online 2009-10 Final Report system is active, you can copy and paste the collected information into the appropriate online forms. You may access the Final Report templates from our website at:

CTE-1 District Certification of Final Report and Claim for Funds form

Data on this form will be prefilled from the certified Final Report on the Title IC Year-to-Date (YTD) reporting system, therefore you will need to complete the YTD Final Report first (see the Final Note below).

CTE-5 District Contact Information Form will be prefilled. Contacts can be updated. No template is provided.

Section I Part C List of TOP Codes & Titles Funded – Final Report

The 3rd column shows the Amendment Status: N=No change to Local App; D=Delete from Local App; A=Add to Local App. If you select either D and/or A; you will be required to complete a Section I Part G. The 4th column “Budget” will be prefilled with the amounts provided in your local application. In the 5th column “Expenditures,” you add the total expenditure for each TOP Code and Across CTE Program funded. Allowable administration expenses charged to the Perkins grant should be included at the TOP Code level only if it is distributed at that level. Districts and colleges using administration at the District or college level should report it as a single entry selected under across CTE Programs

Section I Part G Local Application Amendment Request Form (optional)

Changes selected in the Section I Part C Amendment Status will prefill on this form and you will need to complete the narrative sections as appropriate.

Section II Part A Program Information by TOP Code – Final Report

The first two narrative sections will be prefilled from your 2009-10 Local Application. You will need to complete the Final Report narrative sections as appropriate. For the Nine Perkins Requirements for Uses of Funds: you will indicate whether the requirement(s) reported as UNMET in the Local Application is NOW MET. If you do not select NOW MET, the system assumes the Requirement is still UNMET, and requires you to complete the UNMET narrative section. The Final Expenditure by TOP Code amount is prefilled from the corresponding Section I Part C column 5 entry. In the final section of this form you identify all the Perkins funded activities.

Section III Part A Program Information Across CTE Programs – Final Report

This form is similar to the Section II Part A except there is no section pertaining to the Nine Perkins Requirements for Uses of Funds.

District Audit File Final Report Forms (Not submitted online)

Section II Part B Final Report Program Information by TOP Code Programs -- Final Report

You will update the completed forms in your Local Application, by reviewing Column B, C, and D and entering the numeral in Column E (Status of Activity) that reflects the outcome of each planned activity. If you have added and/or deleted a TOP Code on the Section I Part C form, you must complete this Final Report form for the district audit file.

Section IV Part B Budget Summary by TOP Code -- Final Report

The final expenditures on this form should be identical to the amounts listed in Section I Part C column 5 for each corresponding TOP Code.

Section III Section B Program Information Across CTE Programs -- Final Report

You will update the completed forms in your Local Application, by reviewing Column B, C, and D and enter the numeral in Column E (Status of Activity) that reflects the outcome of each planned activity. If you have added and/or deleted an Across CTE Program on the Section I Part C form, you must complete this Final Report form for the district audit file.

Section IV Part C Budget Summary Across CTE Programs -- Final Report

The final expenditures on this form should be identical to the amounts listed in Section I Part C column 5 for each corresponding Across CTE Program/Project.

*Note: The last three forms are optional budget summary worksheets designed to provide assistance in determining the district final report of expenditures per the five Perkins expenditure A-E categories on the Title IC YTD online reporting system.

Please be aware the due date for your CEO to certify and submit the district’s Final Report online is August 31, 2010. Failure to meet this deadline may result in the forfeiture of 10% of the district’s 2009-10 Title IC allocation. If you have any questions, please contact your CTE Unit Program Monitor. Final Note: While you may utilize the templates until the Perkins Title IC Online Final Report is officially released, a key fiscal part of the 2009-2010 Final Report package is available online now. The Section IV Part A Budget Summary by District Final Report in the Title IC YTD Reporting System at: https://misweb.cccco.edu/vtea/prod/logon.cfm is available once the fourth quarter report is certified and approved. The YTD Final Report Budget Summary report must be certified and submitted online by the district’s Chief Business Officer by August 25, 2010.
