I give permission for my son / daughter to participate in the music trip to Walt Disney World, Florida, planned for April 27, 2011 to May 1st, 2011 (pending flight times). Here is my $200. I understand that all payments are non-refundable.
All-City Members: Please make cheques payable to DBPA.
Prince Andrew Students who are NOT all-city members: Please make cheques payable to Prince Andrew Fine Arts Dept.
Upon accepting your payment, it is expected that students practice their time management skills so they can consistently participate in most / all rehearsals and concerts, particularly as we get closer to the trip. If you cannot make a rehearsal due to unforeseen circumstances, please notify your director(s) ASAP.
