OSHA CONFIDENTIALITY

As required by OSHA (Occupational Safety & Health Administration) regulations, a separate log will be maintained for privacy cases.

If you do not wish your name to be listed on the standard OSHA log you would need to check the "Confidentiality" box under the description section on the "ABX AIR, Inc. Supervisor's Accident Investigation" form.

There are certain instances where your name will automatically be removed from the log, they are:

· An injury or illness to an intimate body part or the reproductive system,

· An injury or illness resulting from a sexual assault,

· A mental illness,

· A case of HIV infection, hepatitis, or tuberculosis,

· A needlestick injury cut from a sharp object that is contaminated with blood or other potentially infectious material, and

· Other illnesses, if the employee independently and voluntarily requests that his or her name not be entered on the log.
