What is Your Life Choices?
Your Life Choices is ConocoPhillips’ integrated and comprehensive package of health improvement programs offered to Plan participants enrolled in the self-insured medical options (HDHP, PPO, EPO and Traditional). These programs are automatically made available to participants on a voluntary basis at no additional cost.
Why is the Your Life Choices program being offered?
ConocoPhillips is committed to helping you make your health a priority. By participating in this program, you can take steps to improve the quality of your life and reduce health care costs.
Why should I participate in the Your Life Choices program?
The more you know about your health, the better prepared you will be to make decisions about how to improve it. By participating, you will have the information you need to take charge of your health.
What are the enhancements coming in 2011?
Beginning Jan. 17, 2011, you and your eligible dependents can choose to participate in two new services:
· Biometric screenings to help you “know your numbers,” and
· A Well-Being AssessmentSM.
In addition, once you participate in these services and have the resulting information in hand, you can opt to take advantage of the newly enhanced support services as part of Your Life Choices, including:
· Web-based Plan for Well-Being
· Online Personal Coaching
· Phone Health Coaching (for those who qualify)
· Resources for Quitting Tobacco
What are the biometric screenings and why are they important?
Biometric screenings include: cholesterol panel (total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides) and blood sugar panel (all of which require nine hours of fasting to ensure accuracy of results), plus blood pressure and body mass index (BMI) (height, weight, hip and waist measurements).
These numbers are important indicators of overall health and can help you know if you need to take steps to decrease your health risks.
How can I get my biometric screenings collected?
Between Feb. 17 and July 31, 2011, you can have your biometric screenings collected at your physician’s office during your annual physical (preventive care visit). Or, for those at work locations with 20 or more employees, screenings will be available onsite during an announced Health Fair or Health Screening. More details will be coming soon.
Who administers the biometric screenings at the work locations?
ConocoPhillips has contracted with a third-party vendor, Onsite Health Diagnostics, to administer the biometric screenings. More details will be coming soon.
When will Onsite Health Diagnostics be at my work location?
Onsite Health Diagnostics will be at locations with 20 or more employees between Feb. 1 and July 31, 2011. More details will be coming soon.
How do I sign up for the biometric screenings?
Participants will sign up through the Onsite Health Diagnostics website. More details will be coming soon.
What if I miss the date Onsite Health Diagnostics is at my work location?
If you miss the date, a form will be made available for you to take to your physician’s office during your annual physical (preventive care visit). More information on the form will be available soon.
What is a Well-Being Assessment (WBA)?
The WBA is a confidential questionnaire about your health and lifestyle. Completing the WBA is an important step because it provides insights into your health, suggests ways to manage health risks and enables you to track progress. Based on your risks, you are introduced to programs to help you improve your health.
When can I participate in the WBA?
You can take the WBA any time between Jan. 17 and July 31, 2011, by logging on to www.yourlifechoices.com. You are encouraged to take the WBA after you have received your biometric screenings.
Who can participate in the Well-Being Assessment and Biometric Screenings?
· All U.S. Active Plan participants (employees and dependents*, including COBRA, disability, personal leaves and hTosco LTD participants) are eligible for the Well-Being Assessment and Health Coaching. *Plan participants must be over 18 years of age to participate in all health coaching programs except for asthma and diabetes.
· Only U.S. Active Plan employees are eligible for the biometric screenings at work locations.
· All U.S. Active Plan participants (including dependents, COBRA, disability, personal leaves and hTosco LTD participants) are eligible for biometric screenings provided at their physician’s office as part of a Preventive Care visit according to regular Plan Benefits. However, only the primary medical card holder needs to complete the Well-Being Assessment and biometric screenings in order to receive the incentive.
What happens after I take the WBA and biometric screenings?
In the event your WBA or biometric screening identifies a potential medical concern, Healthways medical professionals will contact you and/or your health care provider. You will receive a letter or a telephone call inviting you to participate in a health coaching program. If you have caller ID on your telephone, it will read “Your Health Coach”.

What is the incentive?
ConocoPhillips is offering a $600 contribution to help defray your 2012 costs in the existing medical and dental plans for employees (primary medical card holders only) who complete both the WBA and biometric screenings.
What is the incentive for HDHP plan participants?
The 2011 HDHP premium to employees is zero. The 2012 HDHP premium will be determined in mid-2011 and will be set based on 2010 and prior-year claims experience and forecast increases in health care costs that we cannot predict at this time. Based on this, the 2012 HDHP premium may not continue to be zero, but it will be substantially lower than the other options.
How the incentive is delivered to HDHP participants, either through a premium reduction or an additional contribution to the Health Savings Account, is still being determined. HDHP participants will enjoy the same incentive value as participants in other medical options for taking the WBA and biometric screenings.
If my spouse is an employee of the company and covered as a dependent on my insurance, does our incentive double?
There is only one premium being paid per medical policy holder; therefore, only the primary medical policy holder receives the incentive. However, if both you and your spouse are individually covered under “You Only” coverage, then both of you would receive separate incentives as you are separate medical policy holders.
Do I have to participate?
You are under no obligation to participate; however, if you are an eligible employee, a $600 contribution from the company will be offered to help defray your 2012 costs in the existing medical and dental plans for employees who complete both the WBA and biometric screenings.
Is my privacy protected?
Your personal health information is important and should be kept confidential. That is why ConocoPhillips, Onsite Health Diagnostics and Healthways have a strict policy in place to protect your privacy rights. All personal health information that you share through the Your Life Choices program will remain confidential and will only be used as permitted by law. Please take advantage of the well-being improvement tools and support services available to you, confident that your personal information is private and safeguarded from unauthorized access.
