Author Guide

Purpose of the Journal:

Family & Community Health (FCH) is an interprofessional journal that provides a forum to discuss family and community health care, health promotion, and maintenance, innovations, and evidence based approaches in health care. The goal is to discuss ways to reduce the number of illnesses requiring hospitalization and cut the cost of health care. Since many illnesses can be prevented through adequate diet and nutrition, regular exercise, stress management and periodic health appraisals, FCH focuses on these and other issues related to better individual, family and community health. A particular emphasis is given to how individuals, groups, and populations can effectively promote and maintain health. Some of the quarterly issues of FCH focus on a specific topic while others are open to a more general discussion of creative, innovative approaches. Issues contain articles from health care educators, researchers, and practitioners. Each issue is suitable for use as a text or supplement in educational programs such as nursing, social work, health education, medicine, nutrition, and public health. There is a designated Issue Editor for each issue.

Article Selection and Manuscript Review Process: It is understood that articles are submitted solely to this Journal and have not been published previously. All manuscripts are peer reviewed prior to final publication of the article.

Peer Review to Determine the Scholarly Merit of the Article:

All manuscripts are reviewed by at least two peer reviewers and the Issue Editor. Typically, members of the Editorial Board serve as peer reviewers and evaluate manuscripts based on the following criteria:

·  concise, logical ordering of ideas

·  sound argument and defense of original ideas

·  accuracy of content

·  adequacy of documentation

·  consistency with the purpose of the journal


Manuscripts are sent to the reviewers anonymously, with a form for recording their evaluation according to the criteria. The comments for each reviewer are sent to the Issue Editor. The Issue Editor informs the author about the status of the manuscript.

Compliance with NIH and Other Research Funding Agency Accessibility Requirements:

A number of research funding agencies now require or request authors to submit the post-print (the article after peer review and acceptance but not the final published article) to a repository that is accessible online by all without charge. As a service to our authors, LWW will identify to the National Library of Medicine (NLM) articles that require deposit and will transmit the post-print of an article based on research funded in whole or in part by the National Institutes of Health, Wellcome Trust, Howard Hughes Medical Institute, or other funding agencies to PubMed Central. The revised Copyright Transfer Agreement provides the mechanism.

Conflicts of interest

Authors must state all possible conflicts of interest in the manuscript, including financial, consultant, institutional and other relationships that might lead to bias or a conflict of interest. If there is no conflict of interest, this should also be explicitly stated as none declared. All sources of funding should be acknowledged in the manuscript. All relevant conflicts of interest and sources of funding should be included on the title page of the manuscript with the heading “Conflicts of Interest and Source of Funding:”. For example:

Conflicts of Interest and Source of Funding: A has received honoraria from Company Z. B is currently receiving a grant (#12345) from Organization Y, and is on the speaker’s bureau for Organization X – the CME organizers for Company A. For the remaining authors none were declared.

In addition, each author must complete and submit the journal’s copyright transfer agreement, which includes a section on the disclosure of potential conflicts of interest based on the recommendations of the International Committee of Medical Journal Editors, “Uniform Requirements for Manuscripts Submitted to Biomedical Journals” (www.icmje.org/update.html). This form is readily available on the Information for Authors page on the journal website and can be completed and submitted electronically. References:

References must be cited in text and styled in the reference list according to the American Medical Association Manual of Style, ed. 10, Copyright 2009, AMA. They must be numbered consecutively in the order they are cited; reference numbers may be used more than once throughout an article. Do not use ibid or op cit. Page numbers should appear with the text citation following a specific quote.

References should not be created using Microsoft Word's automatic footnote/endnote feature.

References should be included on a separate page at the end of the article and should be double-spaced.

Citation. Each reference should be cited in the text. Citation may also be made in tables, figures and legends. When more than two references are cited at a given place in the copy, use a dash to join the first and last numbers of a closed series; use commas without space to separate other parts of a multiple citation. (As reported previously, 1,3-8,19 )

If you wish to cite different page numbers from a single reference source at different places in the text, include the page numbers in the superscript citation and cite the source only once in the list of references. Page numbers should appear with the text citation following a specific quote. (Jacobsen concluded that “the consequences of muscle strength....”5(pp3,4)

Minimum acceptable data

Journals - Author, article title, journal, year, volume, inclusive pages.

Books - Author, title, place of publication, publisher, year.

Other sources - Enough information so that material can be identified and retrieved.

AUTHOR'S MANUSCRIPT CHECKLIST FOR JOURNALS

Authors should pay particular attention to the items below before submitting their manuscripts.

Manuscript Preparation

Manuscripts should be created on IBM-compatible (PC) equipment using a Windows operating system. Our preferred software is Microsoft Word.

Electronic files should be submitted for all text. Authors have the option of submittinghigh-resolution, camera-readyblack & white artwork electronically using the guidelinesoutlined under"Illustrations." An electronic copy of the Article Submission Form (one for each author) and an electronic copy of the Copyright Transfer Form (can be scanned and emailed)- signed by all authors- should be submitted.

Manuscripts should be double-spaced (including quotations, lists, and references, footnotes, figure captions, and all parts of tables). LENGTH: between 18-24 pages; double spaced, 12 point type; including abstract, tables/figures and references.

Manuscripts should be ordered as follows: title page, abstract, key words, text, references, appendixes, tables, and any illustrations.

Manuscript Contents: Each manuscript must include the following:

1.  Title page including (1) title of the article; (2) author names (with highest academic degrees) and affiliations (including titles, departments, and name and location of institutions of primary employment, and email address); (3) any acknowledgments credits, or disclaimers; and (4) disclosure of funding received for this work from any of the following organizations: National Institutes of Health (NIH); Wellcome Trust; Howard Hughes Medical Institute (HHMI); and other(s).

2.  Abstract of no more than 100 words and 3 to 5 key words that describe the contents of the article like those that appear in the Cumulative Index to Nursing and Allied Health Literature (CINAHL) or the National Library of Medicine's Medical Subject Headings (MeSH).

3.  Clear indication of the placement of all tables and figures in text.

4.  Signed copyright transfer form or U.S. Government Work form (attached). You may use an electronic signature or you may sign, scan and email the form. ALL authors should sign this form.

5.  Completed article submission form for each contributor (attached).

6.  Written permission for any borrowed text, tables, or figures. Most manuscripts do not use borrowed materials.

Illustrations

High-resolution, camera-ready images may be submitted electronically as either a Tagged Image File Format (TIFF) or an encapsulated PostScript (EPS) file in Adobe Illustrator®, Adobe Photoshop®, or QuarkXPress®. Please save files in both the application in which they were created (i.e., Adobe Illustrator®) and as either EPS or TIFF files. Use computer-generated lettering. Do not use screens, color, shading, or fine line. We cannot acceptart that has been photocopied,is embedded in a Word document (has a .doc extension), was downloaded from the internet, is supplied in JPEG or GIFF formats, orwas created in Pagemaker orPowerpoint.A laser proof must accompany artthat is being submitted electronically.

In lieu of original drawings and other material, a sharp, glossy, black-and-white photographic print between 5" x 7 and 8" x 10" is acceptable.

Each figure should have a label on the back indicating the number of the figure, the names of the authors, and the top of the figure. Do not write on the back of figures, mount them on cardboard, or scratch or mark them using paper clips. Do not bend figures.

Cite each figure in the text in consecutive order. If a figure has been previously published, in part or in total, acknowledge the original source and submit written permission from the copyright holder to reproduce or adapt the material. Include a source line. Type “Source: Author” on figures you created. This will help Lippincott Williams & Wilkins identify the status of each figure.

Supply a caption for each figure, typed double-spaced on a separate sheet from the artwork. Captions should include the figure title, explanatory statements, notes, or keys; and source and permission lines.

Provide a camera-ready copy and a separate electronic file for each piece of artwork.

Do not embed art in your text file.

Tables

Tables should be on a separate page at the end of the manuscript.

Cite each table in the text consecutively and number each tables consecutively. Provide a brief title for each.

Include explanatory footnotes for all nonstandard abbreviations.

If you use data from another published or unpublished source, obtain permission and acknowledge fully. Include a source line. Type “Source: Author” on tables that you created.

Permissions

Authors are responsible for obtaining signed letters from copyright holders granting permission to reprint material being borrowed or adapted from other sources, including previously published material of your own or Lippincott Williams and Wilkins’. Authors are responsible for any permission fees to reprint borrowed material. This includes forms, checklists, cartoons, text, tables, figures, exhibits, glossaries, and pamphlet(s); concepts, theories, or formulas used exclusively in a chapter or section; direct quotes from a book or journal that are over 30% of a printed page; and all excerpts from newspapers or other short articles. Without written permission from the copyright holder, these items may not be used.
