Our District election will be held at our annual convention on Sunday, May 1, 2016, at the Grand Sierra Resort in Reno, Nevada.

Your cooperation in filling out and returning this form prior to April 1, 2016, will avoid delays in processing the registration of your club’s official delegates and assuring them their right to vote. Please note the following:
1. Make sure your club dues (MD4/4-C5 and International) are paid and current (contact Cabinet Treasurer John Vermeltfoort if you have a question about your dues).
2. Fill out the enclosed delegate form---PLEASE PRINT THE NAMES.
3. Only the Club President or Club Secretary can sign the delegate form.
4. Mail or email prior to APRIL 1, 2016.
