I hereby declare that I am over 18 years of age and not disqualified by being convicted of any offence at any time under the Protection of Animals Act 1911, the Protection of Animals (Scotland) Act 1912, the Performing Animals (Regulations) Act 1925, the Pet Animals Act 1951, the Animals Cruel Poisons Act 1962, the Animal Boarding Establishments Acts 1967 and 1970, the Breeding of Dogs Act 1976 or of an offence under any sections 4, 5, 6 (1) and (2), 7 to 9 and 11 of the Animal Welfare Act 2006.

I agree to pay the cost of a veterinary officer’s inspection of my premises and understand that this will be charged at an hourly rate and that I will be advised of the total amount due once this inspection has been completed. This will be payable prior to the grant of a Licence.
