The DON LUGO Football Club is hosting a FREE KICKING, PUNTING, AND LONG SNAPPING Skills Camp featuring Luis Zendejas formerly of the Dallas Cowboys and the Philadelphia Eagles, on Saturday, June 23rd, 2012 for any kids of any age at Don Lugo High School (13400 Pipeline Ave, Chino, CA 91710). The athletes will have the opportunity to receive on-field instruction from the Pros. The following session will be offered:

I authorize my child to participate in the Don Lugo Free Kicking, Punting, and Long Snapping Skills Camp on 6/23/2012 (the “Event”). I understand that there are risks and dangers involved in the Event, and I voluntarily assume all of those risks and dangers, including the risks and dangers of personal injury and property damage that may occur during or arise from the Event. In exchange for being permitted to participate in the Event, I hereby expressly and unequivocally release and discharge Z Kick for Life, Don Lugo High School, and their respective officers, directors, employees and volunteers, subcontractors, sponsors, agents, affiliates, subsidiaries and shareholders (collectively the “Releasees”) from all present and future claims that may be made by me, my family, estate, heirs or assigns for property damage, personal injury, or wrongful death arising as a result of my participation in the Event wherever, whenever or however the same may occur. I understand the Event may be photographed, videotaped or otherwise recorded, and I agree to let the Releasees use my name, likeness and demographic information free of charge in any manner and for any purpose in any media now known or hereafter developed.
I am not under any legal disabilities that prevent me from being legally bound by this Waiver and Release of Liability for Personal Injury and Property Damage (this “Release”). I agree that this Release, and any dispute arising from or involving this Release, shall be governed by and construed in accordance with the laws of the State of California.
