2018 HALL OF FAME NOMINATION FORM

OKLAHOMA HIGHER EDUCATION HERITAGE SOCIETY

Each year the Society recognizes and honors individuals, living and deceased, for outstanding achievement in higher education in our state by induction into the Oklahoma Higher Education Hall of Fame. Anyone can make a nomination to the Hall of Fame. Completed nominations on the prescribed forms must be received by the Society no later thanApril 12, 2018.

To be eligible for consideration, nominees must have been employed full-time by an institution or institutions of higher education in Oklahoma as a faculty member, administrator, or staff for a minimum of ten years and there demonstrated excellence in scholarship, teaching, research, administration, outreach, or public service, or any combination thereof, and in doing so brought honor and distinction to Oklahoma higher education. In addition, individuals not so employed but who have performed meritorious service to higher education within the state may be considered for induction.

To begin the evaluation and selection process, this NOMINATION FORM must be completed in full and accompanied by (1) a one-page STATEMENT OF NOMINATION describing the nominee’s distinctive achievement in Oklahoma higher education and (2) a resume not more than 10 pages in length prepared by the nominee or nominee’s institution. While not required, letters of recommendation may be submitted. Not more than three letters of recommendation, none of which may exceed three pages in length, may accompany the NOMINATION FORM and be considered. Incomplete nomination packets will NOT be considered.
