INSTRUCTIONS: Please complete this application and return it to the Financial Aid Office, Columbus State University, 4225 University Avenue, Columbus, GA 31907 or fax to (706) 568-2230 Attn: Financial Aid. All items must be completed unless otherwise indicated.
NOTE: A Federal Direct Stafford Loan can only be processed once you have been accepted for admission and all federal aid application documents received by CSU (Free Application for Federal Student Aid (FAFSA) and any requested verification documents). To be eligible for disbursement of a Stafford loan, you must be enrolled at least half-time (6 hours for undergraduate or 4 hours for graduate) and in an eligible program.

AWARD NOTIFICATION: If you are eligible for the loan program(s) you have requested, you will receive an email at your columbusstate.edu account with award letter information. If this is your first loan with CSU, an additional email notification will be sent describing actions needed for loan counseling and signatures for the promissory note. You will also receive a disclosure statement from Direct Lending.
