Application Checklist
Most recent audit and/or financial statement of applicant
o For nonprofit applicants: Submit audits or auditable financial statements
o For for-profit applicants: Submit current financial statements for the business; profit and loss statements, balance sheets and federal tax returns for three (3) most recent years; and, personal financial statements of all major share holders including personal federal tax returns for three (3) most recent years
Nonprofits only – Proof of Internal Revenue Service designation (IRS Determination Letter)
Articles of incorporation, by-laws, partnership agreement, certificate of good standing from the Secretary of State, and other relevant information regarding legal status
Evidence of property site control
Recent property appraisal
Recent market study to support development proposal
Letters of support, including funding commitment letters from all agencies that will provide funding for the project and resident services, if applicable
Service plan including letters of commitment from service providers if you are requesting funds for an assisted living facility, group home or other special needs housing
Environmental studies, as applicable (Phase I, Lead-Based Paint, Asbestos, etc.)

Working drawings and site plans (must be Section 504 compliant: Section 504 - HUD)
Project Narrative which shall include:
o Description of proposed project, target population and income, strategies for energy efficient construction and cost savings in construction costs and design
o Description of services provided and cost to deliver (if applicable)
o Outcomes and milestones for the project
o Strategies for meeting Green Communities energy efficient design standards
o Description of all services and amenities near the project that are appropriate for the population served

Environmental and Historical Review
Applications for funding are subject to an Environmental and Historical review as required by the federal National Environmental Policy Act (NEPA) and National Historic Preservation Act. This review covers four main areas: Flood, Site Hazards & Pollution, Historical and Noise. Please answer the following questions:
1. FLOOD: Is the project site located in a flood hazard area? Yes No
2. SITE HAZARDS & POLLUTION: Is the project site on or near a geological Yes No
hazard area or archeological or cultural resources?
3. NOISE: Is the project site located within 1,000 feet of a major highway; Yes No
3,000 feet of a railroad; or, 15 miles of a commercial airport
or military airfield?
4. HISTORICAL: Does the project site contain one or more Yes No

Buildings Currently Occupied by Existing Residential or Commercial Tenants or Owners
Applications for funding are subject to the federal Uniform Relocation Act. This requires that existing occupants be notified of their rights regarding relocation of their residence and/or business. Please answer the following question:

application and inform them of their relocation rights. If you have any

Accessibility
For new construction of multi-family projects:
