This worksheet is designed to aid the National Council on Problem Gambling (NCPG) Awards Committee. The contributions to our field are increasing daily and in order for the committee to evaluate a nominee we will need an introduction to the nominee and their contribution, and a reference(s) or cite(s). Please select the award and complete the following form to be submitted to the awards committee. Please make multiple copies and submit a separate form for each of your nominations.

The Awards Committee needs to receive nominations for awards by Monday, May 12, 2008. We have provided criteria for each of the NCPG awards.

Media, Print: "The print article(s) that appeared in the last year that best raised public awareness of problem gambling."

Media, Electronic: "The electronic program(s) that appeared in the past year that best raised public awareness of problem gambling."

Government: "The government (or agency/program) that has demonstrated

superior achievement in addressing issues of problem and pathological gambling."

Industry: "The gaming industry company that has demonstrated superior

achievement in addressing issues of problem and pathological gambling."

Distinguished Program Award: “Awarded in recognition of exceptional effort to establish advocacy for problem gamblers in their state.”

Outstanding Contributor in the Field: The person, institution, organization or company outside of the NCPG structure that has demonstrated superior achievement in addressing issues of problem and pathological gambling

PROCEDURES FOR NOMINATION:

General Information: All nominations for awards must meet individual criteria to be considered. Any nomination that either does not meet criteria, is incomplete or received after due date for nominations may not be considered. All nominations must be faxed or postmarked by the due date. Materials that are required to be included with the nomination must also be faxed or postmarked by the designated date. Nominations must be received in the designated format to be considered. The Awards Committee reserves the right to solicit additional information regarding nominees at any time.

Award winners are selected based on the merits of the nomination materials. It is extremely important that your submission(s) for nomination(s) be complete. Since many of the members of the awards committee may not be familiar with your nominee, it is vital that your nomination materials provide adequate information regarding the justification for the award. This information should include specific examples, verification of facts, letters of support, and any other items necessary to make your case. A poorly crafted or incomplete nomination may cost your candidate the award.


MEDIA, PRINT: The print article(s) that have appeared in the last year that best raised public awareness of problem/ pathological gambling. One award per year maximum will be given.

Criteria:

1)  Only Print Media eligible.

2)  Must be a published article.

3)  Article(s) must increase public knowledge of problem/pathological gambling.

4)  Internet articles are not eligible unless also published in other print source.

5)  Name and date media source and article must accompany all nominations.

6)  Article must be original and not a reprint of previous article.

7)  A copy of the article must accompany the nomination along with the date and place where the article was published.

8)  Recipients will be judged based on the breadth of impact. Local, regional or national articles shall be given equal weight and shall be judged based on impact, quality, and ability to reach gamblers and their families.

9)  All nominees are to be recognized by name at the annual Board of Directors awards event.

MEDIA, ELECTRONIC: TV, radio or Internet presentations that have appeared in the past year that have best raised public awareness of problem/pathological gambling. Multiple awards may be given with emphasis on awarding the best presentation in each of the electronic media.

Criteria:

1)  TV, radio and Internet presentations are eligible.

2)  Only aired presentations are eligible.

3)  PSA’s are eligible if previously aired.

4)  Training films are eligible in previously viewed.

5)  Websites are eligible if created and viewed by the general public.

6)  Presentations must educate the general public on one or more of the following: impact of problem gambling, symptoms of problem gambling, resources available for help.

7)  Media piece must be original and not a reprint of previous piece.

8)  All nominations are to be recognized by name at the annual board of director’s awards event.


GOVERNMENT: The Government (or agency/program) that has demonstrated superior achievement in addressing issues of problem and pathological gambling. This category may include separate awards for state/local government and federal agency. A maximum of two awards per year may be given.

Criteria:

1)  State and local governments must provide groundbreaking legislation, funding, research, organization, etc in their state.

2)  Federal agencies must provide ground breaking funding, research, organization, etc. that will impact the country.

3)  Proof of achievement must accompany the nomination.

4)  A statement of support from individuals, affected groups or other concerned citizens from the state or area affected and a minimum of one member of the board of directors must accompany any nomination.

5)  All nominations are to be recognized by name at the annual board of director’s awards event.


INDUSTRY: The gaming industry company(s) that have demonstrated superior achievement in addressing issues of problem and pathological gambling. This category may include multiple awards and may be divided into subcategories based on nominations to recognize the diversity of achievement.

Criteria:

1)  Only gaming industry corporations or agencies will be eligible. This does include state or federal lottery agencies.

2)  Gaming industry organizations from outside of the United States are also eligible.

3)  Industry nominations must demonstrate a direct or indirect positive impact on problem/pathological gamblers and their families. This includes programs designed to provide prevention or intervention on gambling populations

4)  Proof of achievement must accompany the nomination.

5)  All nominations are to be recognized by name at the annual board of director’s awards event.


DISTINGUISHED PROGRAM AWARD: Awarded in recognition of exceptional effort in a specific area or program that furthers the cause of addressing the issues surrounding the problem gambler and their families. This award will be presented to as many affiliates as meet the criteria for exceptional effort and receive nominations.

Criteria:

1)  Only affiliates in “good standing” with the NCPG are eligible.

2)  Nominations must be made by one or more of the following:

A)  Another affiliate in “good standing.”

B)  An industry, governmental, media or private citizen not directly affiliated with the nominee.

C)  At large members of the NCPG board of directors.

3)  Nominee must have demonstrated initiative or creativity in promoting services or related activities in their state.

4)  Nominee may be nominated for exceptional effort to establish advocacy for problem gamblers in their state.

5)  Nominees will be judged based on their work in relation to the circumstances in their own state and not in comparison to other affiliate states.

6)  Nominations must include: status previous to affiliate intervention, the scope of the intervention, and the impact of the intervention.

7)  All nominations are to be recognized by name at the annual board of director’s awards event.


OUTSTANDING CONTRIBUTOR IN THE FIELD: The person, institution, organization or company outside of the NCPG structure that has demonstrated superior achievement in addressing issues of problem and pathological gambling. This category may include multiple awards and may be divided into subcategories based on nominations to recognize the diversity of achievement. Categories for this award include, but are not limited to, treatment programs, financial counseling, ecumenical work, higher education, nonprofit programs, public awareness, advocacy, family outreach and/or therapy, prison/work release/probation and so on.

Criteria:

1) Any person, institution, organization or company that is not an

Affiliate of the NCPG making significant contributions to any issue related to the NCPG mission can be considered.

2) People, institutions, organizations or companies from outside of the

United States are also eligible.

3) Associates nominations must demonstrate a direct or indirect positive impact on problem/pathological gamblers and/or their families or to an

affiliate of the NCPG or to national NCPG.

4) Proof of achievement must accompany the nomination.

5) All nominations are to be recognized by name at the annual board of Director’s awards event.

6)  An awards subcommittee that includes a member of the awards committee and non board members will make recommendations for this award.

7)  All nominations are to be recognized by name at the annual board of director’s awards event.
