NEW EMPLOYEE CHECKLIST

Employee’s Name

Please date each action as it is performed. Upon completion, the new employee and supervisor sign and date the checklist.

______Supervisor completes “Personnel Transaction” form and mails/emails to Diocesan Benefits Administrator (212 N. San Joaquin, Stockton, CA 95202 or , or fax to 209-373-4281).

______Employee completes and returns to supervisor “Emergency Contact” section on above form.

______Employee completes W-4; Supervisor submits W-4 to payroll (Note: Members of religious congregations must have a “Work Agreement” completed by the Employer and signed by their congregation; members of religious congregations do not complete a

W-4).

______Employer and Employee completes I-9; I-9 is filed in a file separate from

Personnel files.

______Employer discusses Employee Handbook; Employee is given copy of

Employee Handbook.

______Employee signs and returns “Acknowledgement of Receipt of Handbook.” (On back page of Employee Handbook.)

______Employee receives copy of his/her Job Description; Supervisor and Employee discuss key elements.

______Employee receives Employee Safety Information (Injury and Illness Prevention Program).

______Employee receives Division of Workers’ Compensation information and “Notice to Employees”

______Employee receives Continuation of Benefits and Family and Medical Leave information

______Employee receives information about evacuation and emergency response

plans.

______If benefits eligible (30+ hrs/wk), Supervisor discusses insurance benefits with employee;

Employee is provided with benefits packet, which includes enrollment/waiver forms for:

□ Medical/ Vision Ins. □ Direct Deposit

□ Dental Insurance □ Flex Spending

□ Student Verification Form □ Voluntary Life

□ Long Term Disability □ Voluntary Tax Deferred Savings Plan (403.b)

□ Summary Plan Descriptions (SPD located on Diocesan website)

□ Symetra Life Insurance – Beneficiary Form

□ Universal Enroll/Decline Form (if enrolling dependents)

______Employee is given office keys, security codes, etc. as appropriate.

______Supervisor gives employee a tour of the facility and introduces

new employee to work colleagues.

______Employee is introduced through bulletin, newsletter, website, email and/or other

communication mediums.

______As appropriate, Employee receives all information about Safe Environment Program including where to be fingerprinted and how to complete the on line training,

Shield the Vulnerable, as a mandated reporter. All completion forms should be retained in the location’s files.

SCHOOLS ONLY:

______Supervisor and Employee discuss and sign Clute Bill

(Clute Bill is filed in Employee’s personnel file).

______Employee and Supervisor sign contract; Employee receives copy of contract.
