** If a new employee attended NEO within the prior year they do not need to attend the class, however follow these directions and submit the required documents with their personnel paperwork:

·  Give the employee the Employee Handbook and the Safety in the Workplace Handout. Review and explain each with employee.

·  Review and sign the YMCA Buffalo Niagara Rules of Conduct, Receipt of Employee Handbook, & Safety Orientation document. Send form to HR with new hire packet.

·  If required based on position, read and sign the Hep B Vaccination Consent or Declination Form. If

they are not sure, advise to decline at this point. They must notify their supervisor and complete a new form within 10 days if they change their mind. Send the form to HR with new hire packet. Revised 3/18/14
