Position Summary
The Closing the Gap Care Coordinator works within the Central Adelaide and Hills Medicare Local Closing the Gap team to assist Aboriginal people within the region who have a chronic condition to effectively manage their health condition, achieve improved health outcomes and avoid hospitalisation.
Key Responsibilities and Duties
The position has as its key responsibilities:
  1. Work in collaboration with other members of the CTG team to achieve optimal client health outcomes.
  2. Work directly with CTG clients and their GP to achieve the client’s health care management plan.
  3. Facilitate client access to a range of specialist and allied health providers and medical aids relevant to their condition(s), including accessing supplementary services and arranging transport to appointments.
  4. Assist Aboriginal people to understand their chronic health condition and ways to manage it.
  5. Support CTG clients toparticipate in regular reviews by their primary care providers, adhere to treatment regimes, develop chronic condition self-management skills and connect with community support services.
  6. Liaising with the client’s family or carer as appropriate.
  7. Maintain accurate and up-to- date CTG client records.
  8. Working within scope of practice and in accordance with industry standards.
  9. Advocate on behalf of the Aboriginal client to ensure health care services are culturally safe.
  10. Work collaboratively with the services in the local area, including services provided by state or local government and non-government organisations.

Works as a member of the CAHML Closing the Gap team and in collaboration with other CAHML staff

Work Health and Safety
  • The employee will work within the Work Health Safety Act (SA) 2012.
  • The employee will comply with Central Adelaide and Hills Medicare Local’s Work Health and Safety accountabilities, practices and policies.
  • The employee will carry out work in a manner so as not to create a risk or hazard to self, others or the environment.
  • The CTC Care Coordinator is required to undertake a Cultural Awareness program within three months of commencing at CAHML, unless they identify as Aboriginal or have undertaken a Cultural Awareness program within the previous 12 months.

  • The employee is responsible for monitoring their daily work, continuously improving their own practice, and contributing to and participating in quality activities that improve services and systems.

  • The employee consistently shares their knowledge and expertise with the team, and listens to the experience and knowledge of others. The incumbent will follow CAHMLs information management system and regularly measure and report program performance against objectives.

  • The employee will assist with ensuring Privacy and Confidentiality needs are understood, applied and maintained in accordance with the Privacy Policy of CAHML.
Confidentiality
  • Except as required by law, the employee must not disclose, divulge, communicate or otherwise place at the disposal of any third party, in any form or by any means and must keep in strict confidence, now and in the future any Confidential Information of CAHML.
“Confidential Information” includes all information of a confidential nature regarding the past, current and future business interests, client and customer details, system processes and methodologies of CAHML. All issues including (but not limited to) these matters, which are conveyed by CAHML to the employee during the term of employment is regarded as being confidential.
Other Requirements
  • The employee is subject to the relevant Criminal History Checks in accordance with the Credentialing and Screening Policy.
  • Some out-of-hours (including weekend and evening) work and intrastate/interstate travel may be required
  • The employee must possess a current driver’s licence and have access to own vehicle. Mileage is reimbursed by CAHML at the current ATO rate.
  • The employee is required to participate into an Annual Performance Review and Development process.
  • All employees are subject to a three (3) month probationary period.

Qualifications
  • Registered Nurse currently registered with Australian Health Practitioner Regulation Agency

Essential Minimum Requirements
Experience / Knowledge
  • A sound understanding of health issues faced by Aboriginal Australians.
  • Knowledge of and experience in working with people with chronic health conditions.
  • Previous experience working in a health service,either within an acute or primary care setting.
  • Understanding of Aboriginal and Torres Strait Islander health programs and resources.
  • Experience working with Aboriginal people and/or within the Aboriginal community.
  • Experience and knowledge of the primary health care sector.
Personal Abilities / Aptitudes / Skills
  • Good oral and written communication skills including the ability to advocate.
  • Possess sound computer literacy skills in the suite of Microsoft Office applications
  • Ability to deal effectively with sensitive and confidential matters
  • Ability to capture and share clinical information with relevant health care providers, including in electronic format.
  • Ability to work cooperatively in a team environment.

Desirable Characteristics
Experience / Knowledge
  • Experience working with general practice.
  • A keen interest in developing new skills and being part of a progressive learning organisation.
  • Demonstrated ability and experience in working with Aboriginal and Torres Strait Islander peoples.
