Dear Parents/Guardians of Fifth Graders,

As your child is nearing graduation of fifth grade, and you are scheduling annual well exams, please keep in mind the requirements for immunizations and physicals for sixth graders entering Rohrer Middle School:

In compliance with the New Jersey Health and Senior Services the following immunizations will be required of all students entering sixth grade in September 2016:

Meningococcal: 1 dose Tdap: 1 dose given no earlier than 10 years of age

(there must be 5 years between Td boosters)

Please send your child’s school nurse written documentation from your health care provider citing the immunizations listed above, on or before October 10, 2016, OR the date, time and name of the medical professional where your child has an appointment. We are recommending you make an appointment with your health care provider at your earliest convenience. If you do not have health insurance you can use the Camden County Immunization Program, where the vaccinations are free of charge. You can call for an appointment at 856-225-5128.

Students not in compliance by October 11, 2016 will be excluded from school.

If your child is considering playing a sport or joining a school sponsored club/activity, please be aware that a sports physical is required in order to participate in each of these activities listed below. Sports physical forms can be found on my website and are valid for 365 days from the date of the exam.

SPORTS PHYSICALS MAY ONLY BE RECORDED ON THE APPROVED NJ SPORTS PHYSICALS FORM. Our school physician MUST review each sports physical turned in before your child is cleared to participate. To avoid any delays in your child’s participation, please turn in the forms as soon as possible. Students playing sports are also required to complete IMPACT testing. Any documents turned in to your child’s school nurse will be forwarded to the RMS health office at the appropriate time.
