Form Purpose

All undergraduate students applying for the Faculty internship unit ATS3129 (12 credit points) and ATS3935 (6 credit points) are required to complete this form. To obtain course advice, please contact Student Services via my.monash. For more information, see FAQs on ask.Monash.
