Guidance Notes:
Who should consider submitting an abstract for a concurrent session and/or poster presentation?
Health and social service workers across a range of disciplines, and especially:
· Dementia Champions
· SSSC Dementia Ambassadors
· Allied Health Professionals
· AHP Dementia Consultants
· Alzheimer Scotland Dementia Nurse Consultants
· Dementia Specialist Improvement Leads
· Partnership abstracts demonstrating integrated and/or cross sector (public, third, independent) approaches to care and support are strongly encouraged
Successful abstracts will demonstrate:
· Progress made in Acute General Hospitals, Community Hospitals, Mental Health Hospitals, and social services settings which has improved the experience and outcomes of care, treatment and support for people with dementia, their families and carers
· Explicit links to delivering the relevant commitments in Scotland’s 3rd National Dementia Strategy (in particular Commitment 6, 7, 9 and 10)

· How your work is being taken forward in partnership, and demonstrates the meaningful involvement of people with dementia, their families and carers
· How your work is really making a difference – what we are looking for here is evidence of impact and that your efforts are bringing about real improvements in the experience and outcomes for people with dementia, their families and carers
