Title of Study:Evaluation of Professional Judgment impacts on Qualitative Exposure Assessments by Certified Industrial Hygienists
You have been invited to participate in a research project titled “Evaluation of Professional Judgment impacts on Qualitative Exposure Assessments by Certified Industrial Hygienists”. This project is being conducted by Kieran Fogarty and David Huizen. This research is being conducted as part of the dissertation requirements for David Huizen. This consent document will explain the purpose of this research project and will go over all of the time commitments, the procedures used in the study, and the risks and benefits of participating in this research project. The study is designed to determine what tools anindustrial hygienist uses in qualitative exposure assessments. Additionally information will be obtained on what information is most valued by Certified Industrial Hygienists (CIHs) when conducting qualitative exposure assessments. Participants for the study are those CIHs that are certified in comprehensive practice, have a valid email address in the roster from the American Board of Industrial Hygiene (ABIH), and are currently in active practice. Data analysis will take place at Western Michigan University and Grand Valley State University.
This survey is comprised of 18 multiple choice and short fill in responses and will take approximately 10–15 minutes to complete. Your replies will be completely anonymous, so do not put your name anywhere on the survey. You may choose to not answer any question and simply leave it blank. You may choose to stop participating in the study at any time for any reason. If you choose to not participate in this survey, simply ignore this email. Following the link below and completing the survey indicates your consent for use of the answers you supply.
There are minimal risks to you for participating in this study. Survey responses are anonymous; the email and IP addresses of respondents will not be linked to individual survey responses. There is no direct benefit to you for participating in this survey, however results from this study could be disseminated at conferences and possibly a submitted for publication in a professional journal. Thus, industrial hygienists may learn how their professional judgments effect their qualitative exposure assessments.
This consent document has been approved for use for one year by the Human Subjects Institutional Review Board (11/18/2012). Because this is an anonymous electronic survey, you do not need to sign a form to participate in this study. You may print a copy of this informed consent form for your records.
