West Cheshire Care Record (WCCR)

Project Overview

Background

The Western Cheshire Health & Social Care Community, led by the Countess of Chester, submitted a bid to the Safer Hospitals, Safer Wards Technology Fund in July 2013 to establish an electronic West Cheshire Care Record (WCCR) across West Cheshire. NHS England have approved the funding request and the project commenced in December 2013.

The capital funding provision is within 2013/14 and as such it is crucial that this project is implemented within this financial year.

The Project

The project is to develop an integrated record incorporating data from the following organisations:

·  Primary Care (37 GP Practices)

·  Acute Care (Countess of Chester Hospital NHS FT)

·  Mental Health (Cheshire & Wirral Partnership NHS FT)

·  Community (Cheshire & Wirral Partnership NHS FT)

·  Social Care (Cheshire West & Cheshire Council)

·  Tertiary (Clatterbridge Cancer Centre NHS FT).

The WCCR will hold near real time data including medication records, diagnostic results and reports, procedure details, clinical letters, medications, summaries and assessments, appointment/event details (IP, OP, A&E, etc), summary social care records and alerts.

Subsequent phases of the project could include provision of clinical workflow tools to support integrated clinically led service delivery.

This project will build on previous investments to integrate primary care records by adding provider (acute, mental health, community and tertiary) records along with social care records to facilitate the creation of the West Cheshire and Chester WCCR. The health community has previously invested in both the Graphnet CareCentric and the EMIS Web products and has a good base with a wealth of data to start from. The current shared records have brought together extracts from 37 GP practices, has some community record integration and electronic links to the Adastra Out of Hours system.

Project Benefits

Safe and secure sharing of information across health and social care organisations will help improve decision making about a patient’s care by providing secure access to an integrated record at the point of care. It is anticipated that this will enable improvements to be made in terms of quality, clinical outcomes, patient safety and efficiency, additionally it will support improvements in communication and provide new opportunities for collaboration.
