STEP 1: ID Badge
Complete and Return these forms to Dr. Horne
- The non-employee registration form

- Security Department will issue your badge:
- Security is located at the main entrance – your photo will be taken when badge is issued
STEP 2: Obtain CMU/Animal Protocol Access
Complete and Return these forms to Lora
- You are required to see the nurse at Summa Health Corporate Health at NEOMED

- You will need to take proof that you have had a tetanus vaccination within the last
10 years OR she will give you a booster shot. - Have the nurse check off the related checkmarks and sign and date this form.
- OSHA Training& General safety tests
- Mechelle Gehle-Wann will need to sign you up for these.

STEP 3: Add personnel form must be completed

- You also need to complete CITI training, Lora Nicholson will let you know which training sections you are required to take.
CITI TRAINING:
- Visit the CITI web site at
- Create an individual account
- Affiliate with the Northeast Ohio Medical University
- Register as a Researcher
The IACUC will require completion of the following CITI modules:
- Working with the IACUC
- Working with Guinea Pigs
- Working with Rats
- Aseptic Surgery (if applicable)
- Reducing Pain and Distress in Laboratory Mice and Rats
CITI will send the IACUC staff Completion Reports as each module is completed. This information will need to be included on the Addition of Personnel form within the Description of Formal Animal Care and Use Training section (see attached form). The specific titles of the CITI modules as well as the date of completion for each module must be included. Both the participant and the investigator will need to sign/date the form(s). One form is needed for each protocol that individuals will be participating under. Once this is done, the completed form(s) can be brought to the CMU office for processing through the IACUC.
FACILITY TOUR:
In order to work with animals and to gain facility access, individuals will need to complete a facility tour. Please contact Linda McCort, CMU Manager, to schedule a mutually agreeable time for a facility tour. Linda can be reached either by phone at 330.325.6557 or by e-mail
