Whether you are physician referred or self-referred with a physician Rx, we are happy to support and mentor you in your journey to wellness. At Polk Therapy, we are staffed with a terrific, skilled group of professionals who will help you every step of the way. Your therapy will begin with an initial evaluation with one of our licensed physical therapists and will progress with the assistance of our outstanding staff.

Meet the Team

Our Physical Therapists

Rick Todd, PT: Rick is a graduate of the University of Florida and boasts 20 years experience as an inpatient and Home Health PT and Outpatient PT. Rick is the sole owner of Polk Therapy.

Jim Scanlan, PT/DPT: Earning his Doctorate of Physical Therapy at the University of St. Augustine, Scanlan has been practicing PT for 20 years.

Dave Cramp,PT: As a University of Pittsburgh graduate, Cramp is now semi-retired and works part-time at Polk Therapy.

Physical Therapy Assistant

Amber Allen, PTA: works directly with patients to assist in the post-evaluation “Plan of Care”. She holds a Physical Therapists Assistants degree and is licensed in the state of Florida.

Speech Pathologist

Mary Porter, MS, *CCC-SLP: Earned her Master’s degree in Speech Language Pathology from Nova Southeastern University and recently earned her *Certificate of Clinical Competence.

Administration

Sharon Trudeau: Is Polk Therapy’s office administrator. Trudeau is responsible for the overall compliance and function of the Outpatient Rehabilitative Facility.

Terri Phillips: Is our in-house insurance and billing representative. Any questions you have prior to your initial evaluation or during your recovery process may be directed to her.

Taeler Brooker and Jenna Costatino: Are Polk Therapy’s receptionists. They will schedule your Initial Evaluation and follow up visits. They are responsible for collecting co-pays and ensuring digital documentation is up to date.

Therapy Support

Dawn McCoy: Assists our therapist with manual therapy and various technician duties. She holds 15+ years of experience as a licensed massage therapist,

Megan Arrington and Cambell Blaser: Are our therapy technicians. They aid in monitoring your exercises and guiding you through the physical therapy process.

Your Personalized Treatment Plan

Your initial evaluation, which will last approximately one hour, will assess your condition and establish a “Plan of Care” or “Treatment Plan.” Your therapist will then determine how many visits per week and the duration of weeks required for a full recovery based on his findings and in conjunction with your physician’s referral.

After your primary visit, you will be scheduled for a follow-up with one of our three therapists, or our PTA, Amber. Keep in mind that if you have been given a Home Exercise Program, compliance to its guidelines will enhance your progress.

Insurance Information

Because our current healthcare climate involves many facets, it is very important you understand your individual insurance benefits. In some cases, a prior authorization may be required before your evaluation, or visits may be limited due to maximum benefits. Please understand that even the smallest changes could greatly impact the outcome of payment for our services.

Scheduling and Appointments

Your appointments will be scheduled with Taeler or Jenna, which we recommend organizing in advance in order to secure the best time slot according to your availability. Please ensure you have scheduled appointments until your discharge date has been discussed with a therapist.

Each appointment will last about an hour. We ask that you be prepared to pay your co-pay or deductible at the time of service.

A few things you need to know upon scheduling an appointment…

• Our therapists need to be aware of your next referring-physician visit in order to send them a progress report.

• Based on your improvement, you physician will determine if he/she wants you to continue therapy.

• If you are a post-surgical patient, measurements must be taken for a progress report.

• If you are sick on the day of your appointment, please stay at home to rest and recover.

• Please call to cancel any appointments you are unable to keep with as much notice as possible.

• No-show initial evaluation appointments will be subject to a $50.00 fee.

• Our waiting room seating is limited. We ask that anyone who may accompany you to your appointment be conscientious of our need to accommodate other patients in our limited space.

• If there is a time when you would like to involve a family member in your treatment, please inform us in advance and we will be happy to accommodate.
