LOCAL SCHOLARSHIP APPLICATION FORM

The Waukesha Deputy Sheriff Association has established opportunities to receive $1000.00 scholarships for the 2011 graduating class. These scholarships will be awarded to eligible students that are graduating from high school and are a resident of WaukeshaCounty. Please feel free to notice in your student/parent publications of our scholarship opportunity.

Instructions:

  1. Please read the criteria for the application carefully. Complete the attached application form. If possible it is best to type the application. Neatness and correct grammar influence the reader. Use the application to promote yourself. Be specific, explicit and thorough.
  1. To be eligible, you must first be graduating from high school and intend on furthering your education. Second, you must be a resident of WaukeshaCounty. Third, you must maintain a minimum GPA of 2.5.
  1. The student who is awarded this scholarship shall receive a one-time sum of $1000.00 for their freshman year of college.
  1. All applications will be carefully read over by the Scholarship Board and will be decided on the basis of the entire application.
  1. ALL APPLICATIONS MUST BE POSTMARKED BY MARCH 1, 2011. All applications must be mailed and have the proper postage. Do not send certified mail. Do not send to the Waukesha Sheriff Department. Applications not containing all required information will be disqualified. Send completed application to: WAUKESHA DEPUTY SHERIFF ASSOCIATION, P.O. BOX 236, WAUKESHA, WI. 53187.

  1. Those receiving scholarships will be notified by April 1, 2011.

  1. Write one page stating why you have chosen the field of education you have, and the reason you should be the recipient of this scholarship.
  1. List extracurricular activities, honors and awards won, offices held as well as dates held and any community service you were involved in.
  1. Give entire work history, including place of employment, hours worked per week and the type of job performed.

Please attach the following to the application.

  1. A copy of your high school transcript and your last semester schedule.
  1. Submit three letters of recommendation. One letter should be from a community member or employer, and the others can be from a teacher, club advisor or coach. Be sure to choose people who know you well and can give a good appraisal of your strengths. Please allow them enough time to prepare a recommendation letter and enable them to give some thought to their letters. All recommendations must be attached to the application when returned.
