W-6 Player Personnel Form: Creating a document for gathering player information

Goal:

A personnel form is used by organizations to collect vital data about their employees. In this project, you will design a personnel form to collect important information about each of your players.

Skills Utilized:

Use the table feature in Microsoft Word to create a player personnel form

Tactics:

·  Use the Internet to research personnel forms and use them as a guide in creating and designing your own

·  Choose an easy-to-read font with a maximum point size of 10-12 points

Instructions:

  1. Follow the instructions for the Planning Form
  2. Use Microsoft Word, create a new document
  3. Save as 2_ lastname_firstname_W-6 player personnel form template in your Dream Team folder that you have created in your Documents > Focus
  4. Insert your team logo at the top of the page
  5. Use the table feature to create a personnel form with fill-in indicators from the Player Personnel Form Example
  6. Use merge and split cell table features to make your table more attractive
  7. In the top row choose one of your teams colors & choose 80% fill
  8. Format the text, columns, and rows so that the personnel form projects a professional image
  9. Enter the data inside the table

·  Be sure to include the column headings from the Planning Form

  1. Resave the document
  2. Carefully proofread your work for accuracy, design, spelling and grammar
  3. Fill out 5 Player Personnel Forms for your players with their information (hint: you will need to make up their Social Security information & other personal data).

·  Create a hyperlink to the Bank Name (e.g. Key Bank, Bank of America, et al)

  1. Resave the document as 2_lastname_firstname_Last name of the player
  2. Create a folder called 2_lastname_firstname_Player Personnel Forms
  3. Place the Player Personnel Form template & the 5 filled out Player Personnel Forms in the folder
  4. Turn-in electronically to the Inbox
