VILLAGE SENIOR TV LEASE
This AGREEMENT is made between the SOUTH ORANGE MODERATE INCOME HOUSING CORPORATION (“Owner”) with offices at 110 Vose Avenue, South Orange, NJ, 07079 and ______(“Resident”), living at 110 Vose Avenue, Apartment #____, South Orange, NJ, 07079.
TERM
This Agreement is effective the ______, day of ______, 20__ for a period of one year until the ______day of ______, 20___. This Agreement will automatically renew for successive one-year terms unless written notice is provided by Resident to the other party at least thirty (30) days before the end of the current term. The Owner shall have the right to terminate the Agreement with thirty (30) days written notice to the Resident at any time during the Term of the Agreement.
PRICE
The Resident agrees to pay the Owner $29.99 per month no later then the first of the month each month for the term of this Agreement. A $15.00 service charge will be added to any amounts unpaid after the 10th of each month.
The Owner reserves the right to change the price at any time during the course of this Agreement by providing Resident with written notice at least thirty (30) days prior to the change in price. The Resident shall have the option to terminate the agreement effective the day that the price change goes into effect by providing Owner withat least fifteen (15) days written notice in advance of the price change.
DESCRIPTION
The Owner will provide the Resident with the ability to access the Owner’s private cable television system from his/her apartment. At the time of the signing of this Agreement, the channels provided are listed in Attachment A to this Agreement.
The Owner reserves the right to change the programming available through the Owner’s private cable television system during the course of this Agreement by providing Resident with written notice at least thirty (30) days prior to the change in programming. The resident shall have the option to terminate the agreement effective the day that the programming change goes into effect by providing Owner with at least fifteen (15) days written notice in advance of the programming change.
MAINTENANCE
The Resident acknowledges that the Owner is not responsible for the maintenance of the Resident’s cable TV service. The Owner will provide Resident with the contact information for the system provider of the private cable TV system with whom arrangements can be made for repairs of the cable system only. The Owner will not be responsible for replacement of or repairs to the Resident’s television or other related equipment for any reason.
ENTIRE AGREEMENT
This Agreement constitutes the entire agreement between Owner and Resident with respect to the subject matter of this Agreement and supersedes and cancels all prior agreements and understandings, oral and written.
IN WITNESS WHEREOF, this Agreement has been signed on behalf of the Owner and Resident on the dates set forth opposite their names.
