The Utah Cowboy and Western Heritage Museum honors men and women whose lives exemplify the independence and resilience of the people who settled Utah, and includes artists, champions, entertainers, ranchers, writers, and those persons, past and present, who have promoted the Western way of life.

In addition, Artist and photographer nominations may submit two supplemental pages illustrating the work of the nominee for a total of four additional pages).

·  Public Offices Held - List positions held by election or appointment and dates in office.

·  Memberships - List memberships in civic, community, fraternal and/or business organizations on a local, state, national, and international level.

·  Awards and Honors - List major awards and honors received and year of recognition.

·  Published Works or Accomplishments (books, articles, artwork, music, photography).

·  Other pertinent Information and Comments - List items not specifically covered elsewhere, such as biographical information, hobbies, interests, and family data.

Two letters of recommendation from persons active in the field that the individual represents are desirable.

You may submit booklets, materials, brochures, photo copies of clippings and/or other ancillary materials to support the qualifications of the nominated candidate. Please note, however, that the selection committee will initially review ONLY this information submitted above, not to exceed four pages in total (excluding personal and artist/ photographer illustrations).

REQUIRED APPLICATION PROCEDURE

·  Completed nomination should be postmarked or delivered no later than January 31 of each year.

·  Nominations should be submitted unbound, on letter size paper (8 1/2" x 11"). Do not staple or place pages in protective sleeves, and please use a paper clip to keep the packet together.

·  Please type, or neatly print the application using black ink.

·  If more than one person submits an individual for consideration, the submitters will be asked to consolidate the materials into one submission with dual submission signatures.

·  All nominations and materials accepted will be considered the property of the Utah Cowboy and Heritage Museum, and will be kept in the museum's archives, available for public research.

·  Non-selected nominees will be re-entered into the selection process for a total not to exceed three years of consideration. If not selected in the three year process, applicants must wait two years before reapplying.

·  Nominations for any one year are available for public review, but not published.

·  Blanks or missing information in applications will not reject an application, but it will be considered "as is".

·  All Selected and Non-selected applicants will be notified by mail.

·  Inductees or their families may be asked to provide artifacts to the museum. These artifacts will be considered loan items indefinitely until the inductee or family requests them back.

·  Anyone can make a nomination to the Hall of Fame except for members of the Utah Cowboy and Western Heritage Museum staff.

·  No member of the Utah Cowboy and Western Heritage Museum Staff or their immediate family shall be eligible for consideration as a nominee.
