Immunization ReviewInstructions

This document outlinesimmunization tasks and timelines for year 4 students (and year 3 advanced placement students). Note: Follow the timeline to ensure you are able to complete the immunization review and any required new vaccinations before the start of your internship.

The final deadline to submit payment for the immunization review is May 12, 2017. Pay online here via the UBC Dietetics e-payment site.

  1. Locate your childhood and adult immunization records

  • Refer to the table on page 2 below:Immunization Requirements and Recommendations from UBC Student Health Services for a list of documented immunization requirements you need to locate.
  • If you do not have these records at home, the following sources may have records:
  • Your parent(s) or guardian
  • Your family doctor, if their office gave you your vaccinations
  • Your local public health unit where you grew up, if you went to their unit to get your vaccinations or if they came to your school to do them
  • It can take significant time to locate immunization records. Start this process as soon as possible.
  • UBC Student Health Services will review your immunization records. If you cannot obtain documentation to confirm your immunizations, it willbenecessary to complete a full adult series of vaccinations. A full series of vaccinations can take up to 6 months to complete.All vaccines and lab work can be done when you go for your appointment.

  1. Book TWO appointments with a nurse at UBC Student Health Services

  • You will need one 40-minute appointment, and one 20-minute appointment, two to three days apart.
  • Appointments should be booked for March at the earliest and April at the latest.
  • If you book earlier than March, your TB test will be out of date by the time you start internship. If you book later than April, you may not have sufficient time to complete any new immunizations before the start of your internship.
  • UBC Student Health Services is located at M334-2211 Wesbrook Mall. Appointments can be booked by phone at 604-822-7011.
  • If you require any vaccines and lab work, these will be done during the two appointments.

  1. Bring required documents to your first UBC Student Health Services appointment

  • A completed UBC Health Science Students Questionnaire(this will be automatically emailed to you after booking your immunization review appointment)
  • ALL immunization records you have

  1. Pay for the UBC Student Health Services immunization review

  • The cost of an immunization review at UBC Student Health Services is $162, which includes any required immunizations you may need.
  • This payment is made directly to the UBC Dietetics Program (not UBC Student Health Services) as part of the pre-internship requirements tasks. Submit payment online at

Immunization Requirements and Recommendations from UBC Student Health Services
Tetanus/Diphtheria (Td) and Pertussis (Whooping Cough) /
  • Required - Childhood series of 5 doses (or 4 if the 4th dose is administered after your 4th birthday) or an adult series of 3 doses (at 0-1-7 months apart)
  • Required -booster within the last 10 years.
  • Highly Recommended - Td with Pertussis (Adacel).
Note: Adacel has an additional cost of $ 30 as it is recommended, not required.
Polio /
  • Required - Childhood series of 5 doses (or 4 if the 4th dose is after your 4th birthday) or adult series of 3 doses (at 0-1-7 months)
  • Recommended – Adult booster dose

Measles, Mumps and Rubella (MMR) /
  • Required- Two documented doses of MMR.
  • Please note that serology is not accepted by Health Authorities

Chicken Pox (VZV) /
  • Required-Serology proving immunity (anti-VZV IgG) OR two doses of VZV vaccine if negative titres

Hepatitis B /
  • Required-Childhood or adult series of three doses
  • Required-Hepatitis B surface and core antigen ANDa Hepatitis B post vaccine anti-body titre.

Tuberculosis (TB) Screening /
  • Required- TB skin test within 6 months of the start of internship.
  • The TB skin test must not be done within 4 weeks of MMR or VZV vaccine
  • If you have had a previous positive skin test a chest x-ray will be required within one year of the start of internship. UBC Student Health can arrange for this, but if you choose to have this done prior to your appointment at UBC Student Health, please bring a report of this x-ray.
  • If you have a positive skin test, UBC Student Health will refer you to BC Centre for Disease Control TB Control. You will be required to forward a copy of their recommendations to UBC Student Health.
