CONSENT FORM FOR WEDDING CEREMONY

Church wedding, known as holy matrimony, is conducted in the House of God, witnessed in the presence of a congregation and in the presence of the Almighty God. In this happy occasion, an atmosphere of beauty, coupled with purity and godliness, is invoked. Furthermore, solemnity and reverence to God should also be maintained.

We, therefore, ask for the cooperation of the brother and sister in Christ, who wish to be married in the chapel, to adhere to the following church policy, which is intended to protect the sanctity of the church, as well as the consecration of the marriage. Needless to say, many helpers will be participating in the preparation of the wedding. We hope the Couple will share some of the information written in this policy with their relatives and friends who will be helping them to make this blissful occasion a joyful and successful one.

RULES AND REGULATIONS

1.  The bride and groom to be (hereinafter called the Couple) must be True Jesus Church members who have maintained their chastity. They must have attained the legal marriageable age of 18 years or older. If they are underage, they must provide written permission from their parents/guardians before the Church will conduct the wedding for them.

2.  Neither bride nor groom has previously married (with the exception of a widowed member).

3.  Neither bride nor groom has previously divorced.

4.  Neither bride nor groom, after receiving the grace of salvation through baptism, has engaged in any sexual relationship, with each other or otherwise.

5.  Two months before their wedding, the Couple should inform the Church Council of their intention to hold the wedding at the church premises and to complete and sign this consent form.

6.  The Church reserves the right to prohibit any practice the Couple or their families may want to introduce into the church wedding when the Church deems the practice superstitious, non-conforming to Church rules, or contrary to Biblical teachings.

7.  When the ceremony is in progress, the Couple are allowed to have one photographer and one videographer inside the chapel. The photographer as well as the cameraman shall not ascend the podium for their convenience. They should avoid standing in the middle aisle when the wedding march is taking place. They are reminded to keep a safe distance so that they will not cause distraction to the solemnity of the matrimony.

8.  Decoration of flowers and other forms of decorations in the chapel is to be kept to the minimum. No candles are allowed in the chapel. Flowers, confetti, and similar things should not be thrown in the chapel. In order to maintain the chapel's sanctity and cleanliness, preparation of decorative flowers/ornaments may be done in the fellowship hall but not inside the chapel.

9.  The pulpit shall have no excessive decorations.

10.  The Couple are to exercise special care during decoration so that the pews and other church furnitures are protected from damage.

11.  Smoking and alcoholic drinking are prohibited within the church compound.

12.  At the church reception, the church will only provide existing tables and chairs. Foods, beverages, miscellaneous items for the preparation of foods and beverages, and disposable utensils (such as paper cups, towels, chopsticks, forks, spoons, tablecloths, etc.), are to be provided by the Couple. The Couple should inform the Church Council if they wish to use the church kitchen for their wedding reception.

13.  All things that are being used should be treated with respect and care. The Couple are to ensure that the chapel and the fellowship hall are restored to their good and clean condition after the wedding. The decorations inside the chapel should be removed, preferably, on the same day after the wedding. The decorations inside the fellowship hall should also be removed as soon as possible, but no later than the next Sabbath service.
