This letter is to let you know about some important changes to the Dr Bhatt and Dr Benn GP practice where you are a patient.
We want to join together with these two GP practices to become one bigger GP practice.
Dr Thomas GP practice.
Victoria Medical practice.
If we do join together
You can still see the same doctors and nurses you do now.
Opening times will remain the same.
/ You will still get all the support you have had in the past.
/ We have sent you some information with this letter that will tell you why we think making one bigger GP practice will be better for our patients.
To tell us what you think of the three GP practices joining together you can
1: You can fill in the survey we have sent with this letter. You must send it back to us by the 19 December 2017.
/ 2: If you like using the internet you can fill in the same survey online. To do this type in:

3: You can come to the Victoria Road Health Centre and meet staff at two meetings to talk about the changes.
· 7 December 2017 - 6 – 7pm
· 12 December 2017 - 12:30 – 1:30pm
You must book to meet staff to talk about the changes. There are three ways you can do this.

Remember you do not have to do anything. You can just use your GP in the normal way.
When we know what our patients think about things we can decide if joining together to make one big GP is the best thing to do. We will let you know about this early next year.
