This summary is to be submitted to your department chair no later than January 17, 2018, for use in the formulation of his/her written evaluation of your work during 2017 and recommendation regarding your 2018-19 salary. One copy of the summary will be forwarded to the Vice President for Academic Affairs, who will use it in preparing his/her own recommendation regarding your salary. This form is designed to be as flexible as possible. A discussion of your long-term goals would be appropriate in any of the three areas of evaluation. Please address all categories.
