BETTER IN KIRKLEES WELLBEING WORKER: 3 POSTS

Scale Touchstone posts = NJC scale 5, spinal column points 22-25 , starting at £20,253 pa.

Yorkshire Sport Foundation post = YSF band 3 (Development Officer level) points 9 to 15, staring at £20,416 pa, pro rata

Hours 2 x 37 hours per week (Touchstone)

1 x 18.5 hours per week (Yorkshire Sport Foundation)

Days of work are negotiable for part time posts.

This post is funded for 2 years, with the potential to be funded for a further 2 years.

It will be necessary to work flexible hours including some evenings and weekends.

Background

Touchstone has recently been awarded the contract to develop and deliver Better In Kirklees (BiK) – Kirklees’ social prescribing service. We are recruiting a team of talented self-starters who develop, promote and deliver an ambitious and outcomes-focused BIK.

Better in Kirklees (BiK) supports Kirklees’ residents with health and social care needs to be active and involved in the community. It does this by linking service users to a growing network of groups and activities. BiK gives information, support and encouragement. The result is that people have improved health and wellbeing, crises are prevented and independence is promoted.

Purpose of the Role

The BiK Workers will form the backbone of the service. Working alongside a team of fully trained peer volunteers (Peer Champions), you will provide short term support to individuals referred to BiK. You will effectively link people to a broad range of externally provided community based preventative support, which complements the statutory services they receive. You will support groups and organisations, who are part of BiK’s growing membership network, to receive and welcome people with a broad range of health and wellbeing needs. You will develop positive relationships with GPs and GP surgeries, in order to relieve pressure on NHS services.

1.RESPONSIBILITIES

1.1 Take referrals for BiK from a variety of sources, screening and comprehensively assessing people.

1.2 To work in collaboration with service users to enable them to achieve their optimum degree of independence. Support people to identify their aspirations and develop their confidence to set themselves life enhancing goals through a support plan.

1.3 To link service users to community based groups, networks and organisations in order to improve/ maintain their health and wellbeing.

1.4 To build positive relationships with a range of local community groups, networks and organisations, supporting them to welcome and develop appropriate support to benefit BiK service users.

1.5 To work collaboratively within a social prescribing partnership model.

1.6 Work with volunteers and be involved in recruiting and supporting them through training and supervision.

1.7 To assess levels of risk and agree risk management plans, including liaising

with multiple agencies.

1.8 To make appropriate interventions when a service user's support requires it or

may represent a danger to themselves or others.

1.9 To be aware of and work in accordance with Touchstone/ Yorkshire Sport Foundation’s policies and procedures including support planning, assessment, health, safety and safeguarding and these are adhered to at all times.

2. GENERAL

2.1 To work flexibly in accordance with the needs of BiK , including out of hours and weekend work as required.

2.2 To maintain records as required by the BiK Team Manager and other Managers, ensuring that minimum standards are maintained with regards to client records and updating information systems within agreed timescales.

2.3 To provide information and prepare reports about the service and

developments as and when required by line manager.

2.4To work alongside volunteer Peer Champions on a daily basis, and liaise with the BiK Peer Champion Coordinator, as appropriate.

2.5.To contribute towards discussions around Touchstone/ Yorkshire Sport Foundation’s policies and procedures.

2.6.To work at all times as part of a team. This includes working with other staff, attending team and staff meetings and developing a teamwork approach to all aspects of the organisation’s work.

2.7.To be inducted, supervised, performance monitored and appraised in line with Touchstone/ Yorkshire Sport Foundation’s performance management policies and procedures.

2.8.To be responsible for personal learning and development where appropriate and undertake training, both mandatory and optional, to increase knowledge, skills and awareness.

2.9.To participate in the further development of the service and organisation in conjunction with the BiK Team Manager and Senior Management Team, as requested.

2.10.To operate within the aims, policies and practices of Touchstone/ Yorkshire Sport Foundation at all times and to be committed to and promote the organisation’s equal opportunities and anti-discriminatory policies.

2.11.To ensure information is dealt with in accordance with Touchstone/ Yorkshire Sport Foundation’s policies around Confidentiality, Communications, Internet, Email and Telecommunications and steps are taken to ensure that confidential information is secure e.g. service user data.

2.12.To provide information about Touchstone and Yorkshire Sport Foundation, in particular about BiK to people/agencies interested the organisation’s work.

2.13The post holder will be part of the BiK team, but may be required to carry out similar duties in other parts of Touchstone/ Yorkshire Sport Foundation to contribute to the effective operation of the organisation.

2.14.To undertake any other duties as directed by the Team Manager, in line with the responsibilities of this post.
