Tools That Provide/Show Information About the Following

  • When do behaviors occur?
  • Times of day
  • Days of week
  • How long dobehaviors last? (duration)
  • How often do behaviors occur? (frequency)
  • How intense is the behavior? (intensity)
  • Patterns of behavior(Be able to show detailed information about the pattern of behavior.)
  • Consider using information collected from:
  • Commonly used progress monitoring tools
  • Notes collected on typical calendar
  • Classroom behavior management systems (record levels or maintain documentation from the levels attained per classroom behavior management systems)
  • Other commonly kept records (Time out records, ISS records, office referrals, attendance and tardy records, academic progress monitoring/records)
  • Complete ABC forms
  • Use form provided or other version of ABC form
  • These forms should provide detailed information about:
  • What happened before the behavior was observed (Antecedent)
  • What behavior was observed (Behavior)
  • What happened after the behavior was observed (Consequence)
  • There should be ABC forms to document multiple behavioral episodes
  • Contact parent and Other Appropriate people/Agencies and Keep log
  • Keep a log of who has been contacted. The log should show who made contact, how and when the contact was made, and why the contact was made. Any response or outcome that resulted from the contact should also be included in the log.
  • Intervene using SCHOOL RESOURCES and keep log
  • School resources may include:
  • Other teachers/grade level teacher teams
  • Instructional Coach
  • Behavior Support Teacher (BST in middle school and high school)
  • Counselor
  • Administration
  • Staff trained to help with behaviors
  • NCI and/or CPI trained staff
  • PBIS team
  • LEAP team
  • Nurse
  • Social Worker
  • Log should indicate how school resources have been utilized (who was involved, when meetings/conferences were held, what interventions and strategies have been implemented, response of student/impact of interventions and strategies on behavioral patterns)
  • Consult your school psychologist
  • Very important to consult with your psychologist
  • Psychologist needs be aware of emerging behavioral issues
  • The psychologist is also your resource for help with more involved discussions and processes such as:
  • Need for behavior map; how to complete a behavior map
  • Need for FBA/BIP
  • Need for involvement from EC program staff and crisis team (Steps 3 and 4)
