Toddler Cot/Mat Permission Form

A firm cot or 2-inch mat with a sheet and blanket or other suitable covering must be provided for each toddler remaining in the center more than four hours.

At Explore and Discover we provide mats for our toddlers to rest on during our naptime. Parents need to provide their own child’s bedding. Parents are also are responsible for maintaining that bedding.

I give permission for my toddler to use a cot/mat for their naptime at Explore and Discover. This is per child care licensing regulation 7.702.73F.
