SUBJECT: Student Health and Safety Training

As mentioned during the Dean’s Council Meeting0m April 1, 2014, in a recent Trustee’s Hazardous Materials Audit (Reference: Audit Report 13-50 dated December 3, 2013 available at http://www.calstate.edu/audit/audit_reports/haz_mat/2013/1350HMMPOM.pdf), it was noted that the University has not fully complied with the requirements regarding the training of students involved in educational activities. These requirements can be found in Section III.B. of the CSU Occupational Health & Safety Policy - Executive Order 1039 (see http://www.cpp.edu/~ehs/files/occupational/pdf/EO-1039.pdf). Based on this Executive Order, the Campus EH&S Program Administrator has established the following campus requirements concerning the training of students in educational activities.

Students involved educational activities, where there is potential exposure to biological, chemical and/or physical hazards (e.g. agriculture, science and engineering, environmental design laboratories/activities) and this exposure could result in a serious injury/illness are required to receive safety training. A serious injury/illness is defined as an injury/illness that can result in loss of life, loss of limb, a serious degree of permanent disfigurement, impairment sufficient to cause a part of the body or the function of an organ to become permanently and significantly reduced in efficiency or hospitalization for other than observation. For your convenience, the chairs of the departments where there are courses or activities where students could receive a serious injury/illness have been copied on this correspondence. However, there may be other departments with courses or activities were students could receive a serious injury/illness. Colleges and/or Academic Departments will:

·  Develop and implement a student safety training program that informs students of the potential hazards and the safe practices/procedures that must be utilized to avoid injury or illness.

·  Maintain records documenting student safety training for a period of 4 years.

·  Maintain a system or method for resolving training deficiencies. Example, a procedure to address students who miss or fail to complete the training and make sure that they complete the training.

The Environmental Health & Safety Department will provide consultation to Colleges and/or Academic Departments in complying with the above training requirements. However, the primary responsibility for complying with Student Health & Safety Training rests with the college dean, associate deans and faculty under the broad direction of the Provost for Academic Affairs. Colleges will certify to Environmental Health & Safety that they have complied with these requirements regarding Student Health & Safety Training on an annual basis, by submitting written verification from the college dean. The attached “Student Health and Safety Training Certification Form” can be used for this purpose. The initial documentation is to be submitted to the Environmental Health & Safety Department by May 14, 2014. Annually, the University will complete a selective verification of the certification information submitted.

Some possible methods to comply with these requirements are listed below:

·  Provide students with a written handout or other information, and the opportunity to ask questions. Require them to sign a training record stating that they have received and understood the training and will comply with the indicated procedures. The Safety Training Documentation Form for Students which is available on the EH&S Web Site at http://www.cpp.edu/~ehs/files/occupational/doc/TrainingStudent.doc can be used for this purpose. Establish a procedure to prohibit educational participation for students that do not comply.

·  Blackboard or Web Based Training Systems can be used to deliver the training and records maintained in the electronic system. Establish a procedure to place a record hold on students that do not comply.

·  Training can be included in the Laboratory Manual or as part of a Laboratory Lecture. Document that all students have received and understood the training and will comply with the indicated procedures. Establish a procedure to prohibit educational participation for students that do not comply.

Implementation of a student safety training program will provide students with an understanding of the hazards they may be exposed to and the established procedures will assist in the reduction of injuries and illnesses associated with the use of hazardous materials and equipment.
