*Note 1*:At the completion of the exam (Day # 4) participants are to report to their Nurse Managers/Clinical Nurse Specialist on their individual units as scheduled.

•You must complete the evaluation to receive the contact hours.
•You have 30 days from the receipt of the 1st “Time Sensitive” email to complete the evaluation. There will also be a 2nd email, a “reminder” email.
•Click the link embedded in your e-mail, complete survey, & hit “submit”.
•Once evaluation is completed & submitted, an email with the subject “Certificate of attendance for online program: “Program Title”XXX” will be sent to you which has your certificate with contact hours as an attachment.
