THE Y.E.S REPORT CONTENTS AND FORMAT

A Year-end Self-survey (YES) Report is prepared by every organization at the end of the year. The YES Report provides basic information about the organization and evaluation of their projects, strategies and documentation of the organization for the entire school year. The YES Report is graded and has direct numerical bearing on the assessment of organization’s application to accreditation status.

Submission Requirements

The YES Report must be printed on short-sized bond paper with maximum of 25 pages using Century Gothic font style in size ten (10) at single paragraph spacing. The Appendices, which also has a limit of 25 pages, must include the transcribed raw data of chosen Project Evaluation Surveys (as Appendix A). Other documents that the organization considers relevant as evidences to support the self-analysis and evaluation may also be included in the Appendices. A hard and soft copy of the report is required.

The Organization’s Statement should be 1 – 2 pages concerning the preparation for accreditation, relevant background and history, plans, objectives, and the unique contribution of the organization for the Loyola Schools Community. In addition, please describe your reason in applying for accreditation. This may help the accreditation committee evaluate the organization for accreditation.

Special Note: An additional three (3) copies of The Organization’s Statement of Purpose and Contribution should be furnished and submitted separately.

PROJECT EVALUATIONS OF BREAKTHROUGH PROJECTS / ACTIVITIES

The organization is required to submit the (1) the documentation of the answered surveys in transcribed-raw data format as Appendix A and (2)answered project evaluation survey questionnairessubmitted separately in an enclosed short brown envelope for ALL formation and contribution to community and social involvement activities listed above. It is the responsibility of the organization to collate the raw data and submit the document together with the YES Report.

RECOMMENDATIONS

Based on the organization’s own assessment, please write down any recommendations to the organization which you think may be helpful to its development and probably in identifying the future goals.

All organization’s officers or members of the Executive Board should affix their signatures to attest for the factuality and their knowledge of the contents of the YES Report.
