Minutes of Virtual Meeting on Monday 23rd March 2015

  1. WELCOME INTRODUCTIONS

A new member has joined the group : Mr Peter Boyce.

  1. MINUTES OF THE MEETING HELD ON 9th December 2014.

This was agreed to be a correct record of the meeting.

  1. REVIEW OF PROGRESS ON KEY PERFORMANCEINDICATORS (KPI) 2014-15

The practice has met most of the KPI targets discussed at the December 2014 meeting. The following progress has been made on outstanding KPIs from the December meeting:

  • 402 patients (21.5%) between the ages of 40 and 74, with no record of any underlying disease, were provided with a health check and their risk score for developing a chronic illness in the future. Those at high risk were provided with appropriate advice and further treatment. Target of 20% now achieved.
  • 665 (50%) of patients, who have other common diseases such as hypertension, heart disease, obesity etc, have been screened for diabetes. We have met the second target level of achievement of 50%.
  1. REVIEW OF PROGRESS ON PRIORITY AREAS FOR ACTION 2014/15

  1. PROGRESS REPORT ON CARE QUALITY COMMISSION (CQC) FEEDBACK REPORT 2014
  • Practice business plan – still outstanding
  • Staff Appraisals have almost been completed.
  • Carpets in consultation rooms have been replaced with flooring that meets HTM standards.
  • Seating in waiting rooms to include arm rests for patients with poor mobility, on order.
  • Review Cleaning contract – all rooms have a daily and monthly cleaning schedule which can easily be monitored by the person who uses that room. This has resulted in an improvement in the cleaning of the building.

PATIENT FEEDBACK

  • Friends & Family Test – December 2014,January & February 2015 results show that a total of 291 (72%) patients would be very likely to recommend the practice to a friend or family member, 70 (17.5%) likely, 19 (4%) neither likely or likely, 8 (2%) unlikely, 9 (2.3%) very unlikely and 2 don’t know. The majority of negative comments related to flexibility with appointments and access via the telephone. Both these areas have been prioritised for action in the coming year.
  • Ipsos MORI GP Patient Survey – Members were pleased to see the very positive response from patients about the Practice. Telephone access was highlighted as a particular problem at busy times during the week. It was noted that a new telephone system was planned for April 2015 and therefore should be included as a Practice priority for the coming year.
  • Other Patient Feedback (various sources)- Patients have expressed an increasing demand for local Anticoagulation, Family Planning and Yellow Fever Vaccination services. See attached report on progress.
  1. PATIENT ON-LINE ACCESS :

The Practice has now switched on this facility for patients to be able to book appointments, request prescriptions and look at part of their medical records ( medication, immunisation, allergies/sensitivities status) on-line. In order to gain access to these services, patients must present in person to reception with photo ID and they will be given the secure patient website address, a username and temporary password.
