Consent with registration and processing of personal data

I.  Consent with the Processing of Personal Data

TITANS freelancers, s.r.o. with its registered seat at Jégeho 16999/8, 821 08 Bratislava – Ružinov, Company ID: 47047224 (hereinafter the “Company”) informs the Candidate Ing. Jan Novak that it processes his/her personal data pursuant to Article 10 of Act No. 122/2013 Coll. on Personal Data Protection, as amended, in the scope: name, surname, address, business name, address of the registered seat, Org. ID (IČO), date of birth, e-mail address, phone number, age, qualifications, work experience and other data provided voluntarily by the Candidate.

By sending his/her CV, which also includes this consent, the Candidate grants a consent to the Company to process his/her personal data for an indefinite period in the above scope, and to provide, disclose and transfer them across the border to the legal and natural persons that will be interested in the supply of the services provided by the Candidate. The Candidate has the right to revoke his/her consent at any time by delivering a written revocation of his/her consent to the Company’s address.

The purpose of the processing of the Candidate’s personal data is to create an opportunity for the Candidate to conclude a commercial-law contract with the Company, whose subject matter will be the supply of the services provided by the Candidate to a third party, depending on the requirements of third parties that have expressed interest in the supply of the services provided by the Candidate.

II.  Information for the Data Subject Pursuant to Act No. 122/2013 Coll. on Personal Data Protection, as Amended

TITANS freelancers, s.r.o. with its registered seat at Jégeho 16999/8, 821 08 Bratislava – Ružinov, Company ID: 47047224 acts through its executive Ing. Marek Greško (hereinafter the “Authorised Person”) for the purposes of collection and processing of the data subject’s personal data in line with Act No. 122/2013 Coll. on Personal Data Protection, as amended, (hereinafter “APDP”) in the position of

-  data controller (hereinafter the “Data Controller”) for the purposes of processing the collected personal data of the data subject.

Scope and Purpose of Personal Data Processing

In line with APDP, the Data Controller processes the personal data of the Candidate (hereinafter the “Data Subject”) based on his/her consent in the scope: name, surname, address, date of birth, e-mail address, phone number, age, qualifications, work experience and other data provided voluntarily by the Candidate. The purpose of the processing of the Data Subject’s personal data is to create an opportunity for the Data Subject to conclude a commercial-law contract with the Data Controller, whose subject matter will be the supply of the services provided by the Data Subject to a third party, depending on the requirements of third parties that have expressed interest in the supply of the services provided by the Data Subject.

Third parties, Recipients, Third Countries

The Data Subject’s personal data may be provided to the legal and natural persons that will be interested in the supply of the services provided by the Data Subject. The Candidate’s personal data may be transferred across the border, and the Candidate expressly agrees, given his/her interest, with the transfer of his/her personal data to any third country for the above purpose.

Rights of the Data Subject

The Data Subject who provides personal data has the right to request a proof of identity of the Authorised Person collecting personal data, or a credible proof of affiliation of the Authorised Person with the entity on whose behalf the Authorised Person acts. The Authorised Person shall grant such request of the Data Subject without undue delay. The Data Subject has the right to revoke his/her consent with the processing of personal data at any time by delivering a written revocation of his/her consent to the Company’s address. The Data Person has been advised that the consent with the processing of personal data is voluntary. Other rights of the Data Subject are contained in APDP.

Advice to and Declaration of the Data Subject

The Data Subject declares that he/she has been advised of his/her rights in line with Article 28 of APDP. The Data Subject declares that he/she has been fully informed about all matters in line with Article 15 of APDP.

I.  Súhlas so spracovaním osobných údajov

Spoločnosť TITANS freelancers, s.r.o. so sídlom Jégeho 16999/8, 821 08 Bratislava – Ružinov, IČO: 47047224 (ďalej len „spoločnosť“) informuje kandidáta Ing. Jan Novak, že jeho osobné údaje spracúva podľa § 10 zákona č. 122/2013 Z.z. o ochrane osobných údajov a o zmene a doplnení niektorých zákonov vrozsahu: meno, priezvisko, obchodné meno, bydlisko, miesto podnikania, IČO, bankové spojenie, dátum narodenia, e-mailová adresa, telefónne číslo, vek, kvalifikácia, pracovné skúsenosti aďalších údajov, ktoré kandidát dobrovoľne poskytne.

Kandidát zaslaním svojho životopisu, ktorého súčasťou je aj tento súhlas, týmto dáva spoločnosti súhlas, aby po dobu neurčitú spracúvala jeho osobné údaje vo vyššie uvedenom rozsahu, tieto poskytovala, sprístupňovala acezhranične prenášala právnickým afyzickým osobám, ktoré budú mať záujem ododanie služieb poskytovaných kandidátom. Kandidát má právo kedykoľvek svoj súhlas odvolať, ato doručením písomného odvolania svojho súhlasu na adresu spoločnosti.

Účelom spracúvania osobných údajov kandidáta je vytvorenie príležitosti pre kandidáta uzavrieť so spoločnosťou obchodnoprávnu zmluvu, predmetom ktorej bude dodanie služieb poskytovaných kandidátom tretej osobe, ato vzávislosti od požiadaviek tretích osôb, ktoré prejavia záujem ododanie služieb poskytovaných kandidátom.

II.  Informácie pre dotknutú osobu vzmysle zákona č. 122/2013 Z.z. o ochrane osobných údajov a o zmene a doplnení niektorých zákonov

Marek Greško (ďalej len „oprávnená osoba“) za účelom získavania a spracúvania osobných údajov dotknutej osoby vzmysle zákona č. 122/2013 Z.z. oochrane osobných údajov aozmene adoplnení niektorých zákonov vplatnom znení (ďalej len „ZOOÚ“) vpostavení

-  prevádzkovateľa (ďalej len „prevádzkovateľ“) za účelom spracúvania získaných osobných údajov dotknutej osoby

Rozsah a účel spracúvania osobných údajov

Prevádzkovateľ spracúva vzmysle ZOOÚ osobné údaje kandidáta (ďalej len „dotknutá osoba“) na základe jeho súhlasu vrozsahu meno, priezvisko, bydlisko, dátum narodenia, e-mailová adresa, telefónne číslo, vek, kvalifikácia, pracovné skúsenosti aďalších údajov, ktoré kandidát dobrovoľne poskytne. Účelom spracúvania osobných údajov dotknutej osoby je vytvorenie príležitosti pre dotknutú osobu uzavrieť sprevádzkovateľom obchodnoprávnu zmluvu, predmetom ktorej bude dodanie služieb poskytovaných dotknutou osobou tretej osobe, ato vzávislosti od požiadaviek tretích osôb, ktoré prejavia záujem ododanie služieb poskytovaných dotknutou osobou.

Tretie strany, príjemcovia, tretie krajiny

Osobné údaje dotknutej osoby môžu byť poskytnuté právnickým afyzickým osobám, ktoré budú mať záujem ododanie služieb poskytovaných dotknutou osobou. Osobné údaje kandidáta môžu byť cezhranične prenášané, pričom kandidát vyslovene súhlasí s tým, aby vzhľadom na jeho záujmy mohli byť jeho osobné údaje za vyššie uvedeným účelom prenášané do akýchkoľvek tretích krajín.

Práva dotknutej osoby

Dotknutá osoba, ktorá poskytuje osobné údaje, má právo vyžiadať preukázanie totožnosti oprávnenej osoby, ktorá získava osobné údaje, alebo preukázanie príslušnosti oprávnenej osoby hodnoverným dokladom k tomu subjektu, v mene ktorého koná. Oprávnená osoba je povinná takejto žiadosti dotknutej osoby bez zbytočného odkladu vyhovieť. Dotknutá osoba má právo kedykoľvek svoj súhlas so spracovaním osobných údajov odvolať, ato doručením písomného odvolania svojho súhlasu na adresu spoločnosti. Dotknutá osoba bola poučená, že poskytnutie súhlasu so spracovaním jej osobných údajov je dobrovoľný. Ďalšie práva dotknutej osoby sú uvedené v ZOOÚ.

Poučenie a vyhlásenie dotknutej osoby

Dotknutá osoba vyhlasuje, že bola poučená o svojich právach v zmysle ustanovenia § 28 ZOOÚ. Dotknutá osoba vyhlasuje, že bola vplnom rozsahu informovaná ovšetkých skutočnostiach vzmysle ustanovenia § 15 ZOOÚ.

Jan Novak zaslaním svojho životopisu spoločnosti vyjadrujem svoj súhlas so spracovaním osobných údajov, ako aj potvrdzujem, že som sa oboznámil(a) so všetkými informáciami uvedenými vo vyššie uvedenom článku II. Dátumom udelenia môjho súhlasu je dátum odoslania môjho životopisu spoločnosti.
