We are kicking off the 27th year of recognizing volunteers through the Someone Special Volunteer Program. The United Way of Bemidji Area, Ken K. Thompson Jewelry, KB101 Radio, and The Bemidji Pioneer are once again sponsoring the program! Take advantage of this opportunity to thank your “special” volunteers who add so much to your organization. Let’s show our volunteers just how “special” they really are!

DIRECTIONS: To recognize your outstanding volunteer(s) with the Someone Special Award, please complete the attached document for each volunteer. You may recognize a maximum of two (2) volunteers. Please use a separate form for each volunteer.

The preferred method for completing the form is electronic format:

·  Use the form on the next page or download from www.UnitedWayBemidji.org/someone-special-volunteers

·  Save the downloaded form to your computer and complete the form. Please save the file as your volunteer’s first and last name (for example: jennifersanford.doc)

·  Email completed form to . Please list “Someone Special Volunteer” on the email subject line.

·  DON’T FORGET to attach a photo of each of the volunteers you are recognizing (if available).
The photo will be included in the Bemidji Pioneer article and posted to United Way’s Facebook Page.
Please name the photo with the volunteer’s first and last name (ex. jennifersanford.jpg)

·  You may recognize up to two volunteers to receive the Someone Special award. You may attach two separate recognition forms and two photos to the email.

IN THE ORDER THEY ARE RECEIVED, ALL VOLUNTEERS SUBMITTED FOR THE SOMEONE SPECIAL VOLUNTEER AWARD WILL RECEIVE THE FOLLOWING:
·  recognition on the KB101 “Morning Show” with Todd & Heidi
·  an article and picture in the Sunday Bemidji Pioneer
·  an engraved personalized plaque from Ken K. Thompson Jewelry (it’s your responsibility to pick this up)
·  recognition on the United Way of Bemidji Area web site and Facebook Page
*Organizations will be notified in advance as to when KB101 and The Bemidji Pioneer will feature their nominated volunteer and when the plaque from Ken K. Thompson will be ready for pick up.
PLEASE NOTE THAT UNITED WAY DOES NOT NOTIFY VOLUNTEERS OF THEIR RECOGNITION. THAT IS THE RESPONSIBILITY OF THE NOMINATING ORGANIZATION.
