PATIENT SURGERY INFORMATION PACKET

PRE-OPERATIVE APPOINTMENT(if applicable)

Pre-operative appointments may take 2-4 hours. During your pre-operative appointment we will complete a history and physical examination, review lab work and imaging (if applicable), and answer any questions you may have about your surgery.

You must bring any required medical clearances to the pre-operative appointment. If medical clearance is not available, your surgery will be rescheduled.

If you fail to keep your pre-operative appointment, your surgery will be rescheduled.

POST-OPERATIVE APPOINTMENT

Upon discharge, you will be informedwhen you will need to follow-up with your surgeon. Please call to schedule your post-operative appointmentat the location in which you are normally seen:

If you need any forms completed by your physician FMLA/INSURANCE FORMSthey must be submitted (2) weeks prior to the surgery and include a $25.00 processing fee. No processing fee for return to work/school forms. Original forms must be hand delivered or mailed with payment. Forms received without payment will not be processed.

PRE-OPERATIVE INSTRUCTIONS

Please review the following instructions to assist you prior to surgery:

  1. Medication Guidelines
  1. YOU MUST BE OFF ALL ASPIRIN BASED PRODUCTS AND NSAIDS 10 DAYS PRIOR TO SURGERY UNLESS APPROVED BY YOUR SURGEON. These medications increase bleeding risk during surgery. (Please see attached list for common NSAIDS and Aspirin containing products.)
  1. YOU MUST STOP VITAMIN E AND ALL HERBAL SUPPLEMENTS 2 WEEKS PRIOR TO SURGERY UNLESS APPROVED BY YOUR SURGEON.
  1. YOU MUST STOP ANY DIET PILLS 4 WEEKS PRIOR TO SURGERY.
  1. If you are blood thinners (see medication list attached, section five), these may need to be discontinued. Please discuss with your surgeon if you are taking any of these medications. You must also inform the physician prescribing the blood thinner that you will be undergoing surgery, and that you need his/her clearance to temporarily discontinue the blood thinner.
  1. You may take Tylenol prior to surgery.
  1. Take your usual prescription medications on the morning of surgery, unless otherwise instructed. Take these medications with only a small sip of water.
  1. All patients will be scheduled for a pre-operative appointment in the office or by phone. If you are scheduled for phone pre-operative appointment, you will be contacted by Tampa General Hospital or Morsani Surgery Center the day prior to surgery to complete the pre-operative process.
  1. If you are required to be medically cleared by another physician prior to surgery, this report must be faxed to us prior to your pre-operative appointment. Please fax your clearance to (813) 974-7314. PLEASE bring a copy of the clearance to your pre-operativeappointment. If we do not receive your clearance, your surgery will be rescheduled.
  1. You will get a call after 1:00 pm the day before surgery to confirm your surgery time and arrival time.
  1. If you are a dialysis patient, please have your dialysis center contact Tampa General Hospital dialysis center to provide orders for your treatment and to facilitate arrangements.You can contact Tampa General Hospital Dialysis Center at (813) 844-7372.
  1. If you are coming from another city, you can contact Guest Relations at Tampa General Hospital at (813) 844-7000 to assist you with information about accommodations. Due to insurance guidelines and room availability we are unable to admit you prior to your surgery.

  1. For Adults and children over 4 years old, nothing to eat or drink after midnight the night before surgery.
  1. For children under 4 years old:

-Clear liquids must be stopped 2 hours before surgery.

-Breast milk must be stopped 4 hours before surgery.

-Cows’ milk and formula must be stopped 6 hours before surgery.

-Solid foods must be stopped 8 hours before surgery.

  1. Take a shower the evening or morning before your surgery. You should wear freshly laundered, loose-fitting clothes.
  1. You should make arrangements to be driven home after surgery. If we need to call for your ride after the procedure, please bring the phone number and name of your driver. You may not leave the hospital after surgery unaccompanied.
  1. Family members/friends who accompany you are welcome to wait in the waiting area; however, they will not be permitted in the operating room during the surgery. The operating room staff will notify them on the progress of your procedure.
  1. Please notify us if you get sick (a cold, fever, rash, cough, or any other medical problem) prior to your surgery. Please call your surgeon’s assistant who scheduled your surgery.
  1. If you have any further questions, please call your surgeon’s assistant who scheduled your surgery.
  1. Failure to adhere to these guidelines will lead to cancellation and rescheduling of your surgery.
