Working to successfully provide healthcare can be both challenging and very rewarding and our Community Services are developed to meet the needs of the local population. The service has had investment to expand and to provide annew integrated community stroke rehabilitation model including early supported discharge.
A exciting opportunity has arisen to work as an assistant practitioner in a dynamic community stroke service working in a variety of settings in the community including home environment, intermediate care setting and residential homes. The Community Stroke Rehabilitation Team provides an urgent and timely community response, from a clinically led inter-disciplinary team to facilitate timely discharge from secondary care, and continue to work with patients to progress their longer term rehabilitation goals.
You will work with an interdisciplinary team of Physiotherapists, Occupational Therapists, nurses and Speech and Language Therapists, to provide rehabilitation packages that facilitate improvements in the patient’s functional ability and independence in the community setting, supporting the rehabilitation process. The team work closely with the Stroke unit, primary and community care, Health Improvement services and the voluntary sector.
The successful applicants will need to be flexible, have relevant experience in working as a assistant practitioner in a rehabilitation setting, possess excellent communication skills and be able to actively contribute to team working. You will be supported by the senior staff and the Rehabilitation Services Manager and will have opportunities for training and development within the Trust.
