I hereby consent to my son’s/daughter’s/ward’s participation on one of FZY’s programmes for the summer 2012.
I understand that acceptance on the programme will be determined by a school reference and medical application and review of behaviour and participation on previous FZY programmes. The information received will be kept confidential and no candidate will be eliminated from participation solely on the basis of a school reference. If my child’s application is not accepted my deposit will be returned in full.
I further understand that should my child/ward be unruly or breach discipline rules then he/she will be returned to Britain at my expense.
Furthermore places on FZY Hanhaga 2012 are limited. Completion of this form does not guarantee acceptance.
Cancellations will be subject to £75.00 administration fee before 11th April 2012. Deposits are non-returnable after this date. The balance must reach FZY by 4th May 2012. I understand I will have the right to cancel and will receive a full refund, less the deposit up until 4th May 2012.
The cost of the programme includes all meals and board.
All correspondence will only be sent to the applicant’s home address and primary email as indicted overleaf.
The Federation of Zionist Youth reserves the right to change dates, prices and content of our programmes.
Please note that this year we shall only be accepting Hanhaga applications and deposits that are sent by mail. Personal deliveries will not be accepted.
