Achievement History Questionnaire

Question One

The Unit Supervisor functions as one of Dane County’s liaisons with the State of Wisconsin regarding the Community Options Program, Medicaid Waiver programs and other State/Federal funded programs. This position is also the liaison with the county’s Purchase of Service COP Case Management Agencies. Please describe in detail your experience in managing and operating the Community Options Program and Medicaid Waivers and working with State of Wisconsin staff related to both of these programs. As the liaison, describe your knowledge and experience working directly with purchase of service case management agencies. Please also describe your experience as the lead staff person in organizing work groups and providing directions/guidance to purchase of service agencies. Please describe the outcome of your relationship with your state, federal and county contacts.

Question Two

The Long Term Support Supervisor provides supervision to assigned case management staff by providing recommendations for hiring staff, conducting performance evaluations, providing training and orientation, involvement in the grievance process, etc. Please describe your experience in managing and supervising subordinate employees and your role in hiring, training, discipline, conducting performance evaluations, coaching, leading, motivating, directing and controlling workflow. Be specific as to the number and job types of subordinates. If you were not a supervisor, please describe any lead work experience you have in directing and supervising staff or a workgroup. Please include any noticeable accomplishments you achieved in a supervisory capacity or as a lead worker.

Question Three

This position is responsible for working with frail elderly and physical disabled individuals. Please describe in detail your experience and knowledge of working with older adults and physical disabled individuals. Please also explain your experience with businesses and community-based agencies providing resources and support to frail elderly and persons with physical disabilities.

Question Four

This position is responsible for understanding the requirements and complexities of the state, federal and county programs. Please explain your knowledge and experience in operating the Community Options Program, Medicaid Waiver programs, Medicaid programs and Supportive Home Care services. Please also describe your experience in training case managers and community-based agencies on the program guidelines governing these programs.

Question Five

The State of Wisconsin expects to transform long-term support programs into a managed care system within the next few years. Describe your role and experience in transitioning an operational system through a major change and in supporting staff during a period of uncertainty and changes as a unit supervisor. What steps would you take in transforming COP and Medicaid Waiver programs into a managed care system?
