The BRIDGES Tutoring program provides ongoing assistance in core courses for students at-risk of failure.

Tutoring is held every Tuesday and Thursday from 3:50p.m. - 5:00p.m.

Tutoring for the Spring 2013 semester begins on Tuesday, January 29th and ends Thursday, April 25th 2013.

Tutoring provided by BRIDGES Smaller Learning Communities Grant funded by the US Dept. of Ed.

Transportation

__ I will ride the bus home after tutoring on Tuesdays and Thursdays.
__ I will provide my own transportation home after tutoring. **Car riders must be picked up by 5:15 in the front drive. Failure to abide by this expectation will result in loss of tutoring privileges**

Procedures

1.  Fill out this form completely and return it to Ms. Peavy in room 14. Students will be placed in tutoring classes on a first-come, first-served basis. If the tutoring class you need is full, you will be placed on a waiting list.
2.  You will receive a confirmation of your acceptance in the program from Ms. Peavy or your classroom teacher. At that point, you may stay after school for tutoring. Please see Ms. Peavy if you do not receive a confirmation slip within a week of submitting your application.
3.  Upon acceptance, students will report directly to their assigned tutoring classroom by 3:50. Students arriving after 3:50 without a written pass will not be allowed to enter the classroom and will not be allowed to ride the bus. They must call home for a ride.
4.  Regular attendance is required to remain in a tutoring class. After 3 unexcused absences, students will be dismissed from the program. Students must also adhere to all school policies and procedures while attending BRIDGES Tutoring. Inappropriate behavior will result in immediate dismissal.

Permission & Agreement

I agree that my student will attend after school tutoring on a regular basis throughout the tutoring program dates listed above. After three unexcused absences, my student may lose their tutoring privileges. Appropriate behaviors and work habits are required for attendance in the tutoring program. I understand that my student is responsible for all Douglas County Schools policies and procedures during tutoring sessions. I also give permission for the release of Directory Information.
