CREATE Center for the Arts Palm Desert is accepting up to 10 submissions per artist. Size limit may NOT exceed 16x16 in (this includes frames). CREATE Center for the Arts Palm Desert reserves the right to refuse acceptance of any work not suitable for display in the gallery. All artwork MUST BE dry and ready for hanging with wire (no hooks, clasps or metal teeth brackets please)!Please read this form in its entirety and be aware of the terms and the pick-up dates before signing this entry form!

• This agreement applies only to work consigned under this agreement and does not make CREATE Center for the Arts a general agent for other works.

• Artist reserves the copyright and all reproduction rights to this work.

•Consignment arrangement is as follows: Artist 50%, CREATE Center for the Arts Palm Desert 50%. ______(initial)

•OR 100% Donation to CREATE Center for the Arts Palm Desert ______(initial)

•Insurance protection is the responsibility of each artist. CREATE Center staff and/or volunteers will not be responsible for loss or damage however caused.

•Artist gives CREATE Center for the Arts Palm Desert the right to photograph the artist and/or artwork for general purposes. This does not include rights to the artwork itself, nor does it allow CREATE Center to reproduce such work for sale or for profit.

•Artist assumes full responsibility for delivery and pick up. All unsold artwork MUST be picked up, by the artist, on the following given dates:

Artwork pick up dates: Mon. Dec 5or Tues. Dec. 6th, 2015 from 10am-5pmor Sat, Dec. 10th, 2015 from 10am-2pm

All artwork that is NOT picked up by December 10, 2016 will be donated to a charity of the gallery’s choice.
