How to Report Workers’ Compensation Claims?

To report via the Internet: Log on to the Crum & Forster website at http://one.cfins.com. Use your assigned User ID and Password. If you do not have a current User ID, please contact your local Crum&Forster Claims or Marketing resource.

What forms and pamphlets does the employer need to be aware of?

Form ICPN – Workplace Notice (Posting Notice)

Prominently display the notice where each employee is likely to see the notice on a regular basis. The Illinois posting notices are available on the Crum Forster website. Notices should be displayed in English and Spanish.

Form IC45 - Employer’s First Report of Injury

Employers shall report to the Commission all injuries resulting in the loss of more than three scheduled work days. Filing this form does not affect liability under the Workers’ Compensation Act and is not incriminatory in any sense. If you report the claim via the Internet or by telephone, you do not need to fill out this form. If you report the claim via fax, complete this form, and fax it to Crum Forster at the fax number provided above.

What is the Waiting Period For Lost Wages in Illinois?

The waiting period is 3 scheduled work days. If the employee is off 14 or more calendar days, the first 3 days are paid retroactively.

What are the Choice of Physician Rules in Illinois?

The employee has the choice of treating doctor. The employer’s responsibility is to pay for all first aid and emergency services, two treating physicians of the employee’s choice and any additional medical care providers to whom the employee is referred by the two treating physicians. Insurance company has right to Independent Medical Exam.

·  Select the “PPO Lookup” box under “Connect”

·  Select the “Search” button under “Find a Provider”

·  Using the drop down option, select “Workers’ Compensation” in the “Select a Network” box

·  Fill in or select from the drop down options for the remaining fields. Make sure you limit your search by selecting the provider type for specialty or service

·  When you have completed all the files, select “find providers that meet the search criteria” to obtain a list of providers meeting your search criteria
